伴不同程度绒毛膜下血肿先兆流产的妊娠结局比较

摘　　要：目的：探讨先兆流产合并不同程度绒毛膜下血肿对妊娠结局的影响。方法收集自2013年1月～2014年12月在潮州市妇幼保健院及潮州市人民医院诊断为先兆流产合并绒毛膜下血肿并住院的孕妇116例，将其设为观察组，按照 B 超下血肿程度分为轻型、中型、重型3组，并以同期收入院的单纯先兆流产孕妇116例为对照组，比较两组孕妇阴道出血持续时间、住院时间、妊娠结局和新生儿出生情况。结果观察组孕妇住院时间和阴道持续出血时间较对照组孕妇长，而新生儿体重和身长均不及对照组，差异均具有统计学意义（P ＜0.05）；不同程度绒毛膜下血肿对妊娠结局无影响，差异无统计学意义（P ＞0.05）。结论与单纯先兆流产孕妇相比，先兆流产伴绒毛膜下血肿孕妇阴道流血时间及住院时间较长，而不同程度血肿对妊娠结局影响意义不大。Objective To explore influences of threatened abortion with subchorionic hematoma in various degrees on pregnancy outcome. Methods 116 pregnant women who were diagnosed with threatened abortion with subchorionic hematoma in our hospital and received hospitalization from January 2013 to December 2014 were allocated to the observation group, and they were divided into light group, medium group, heavy group according to hematoma in various degrees by B-ultrasound examination. 116 pregnant women who were diagnosed only with threatened abortion and received hospitalization in the same period were allocated to the control group. Duration of vaginal hemorrhage, hospitalization time, pregnancy outcome and neonate’s condition in these two groups were compared. Results Duration of vaginal hemorrhage and hospitalization time of pregnant women in the observation group were both longer than those in control group while neonatal weight and length in observation group were inferior to those in control group. Differences of the above data between the two groups were all statistically significant （P 〈 0.05）. No influence of subchorionic hematoma in various degrees was exerted on pregnancy outcome and the differences between the two groups were not statistically significant（P 〉 0.05）. Conclusion Duration of vaginal hemorrhage and hospitalization time of pregnant women who are with threatened abortion with subchorionic hematoma are longer than those only with threatened abortion. Hematoma in various degrees has no significantly influence on pregnancy outcome.
