中医药治疗抗甲状腺药物引起白细胞减少的Meta分析

摘　　要：目的：通过Meta分析评价中医药治疗抗甲状腺药物引起白细胞减少的研究质量和疗效优势。方法：参考Cochrane协作网系统评价的方法，检索4个数据库中有关中医药治疗抗甲状腺药物引起白细胞减少的临床随机对照文献。根据纳入标准，由两位评价员独立进行文献筛选、资料提取和交叉核对。采用Jadad评分标准评价文献质量，疗效判定采用软件RevMan5．2进行Meta分析。结果：共纳入了6篇中文文献，合计394例甲亢患者。结果显示，中医药治疗组与西医治疗组相比较，有效率[OR=1．24，95％CI（1．12-1．38）]及治疗后白细胞水平[MD=0．51，95％CI（0．26～0．76）]，差异有统计学意义。结论：现有有限证据表明，与单纯西医治疗组比较。中医药治疗能一定程度地改善抗甲状腺药物引起的白细胞减少，由于纳入文献等级偏低，尚需要设计多中心、大样本的随机双盲对照试验，并加强中医药疗效评价指标研究以提高证据强度。Objective： To systematically assess the clinical effectiveness of TCM in the treatment of antithyroid drug-induced leukocytopenia by meta-analysis. Methods： According to Cochrane network system, random controlled trials （RCTs） were retrieved from the four databases. Trials were selected according to inclusion criteria. Two estimators independently filtered the papers, extracted the information and checked across. Valuing literature quality in accordance with Jadad＇s score scale and judging effectiveness by the software of Revman5.2. Results： There were 6 trials included, involving 394 patients. Compared with control groups, TCM therapy showed a better effective rate [OR=1.24,95% CI（1.12-1.38）] and more significantly improvement in the level of WBC [MD=0.51,95%CI （0.26-0.76）]. Conclusion： The current limited evidence showed that TCM therapy shows a superior effective rate and better improvement of WBC level, compared with those in western agents. However, further strictly designed multi-center, large samples and double blind randomized controlled trials are required because of the poor quality of included trials and to reinforce the research of TCM effectiveness estimate indexes to get more authority evidence.
