CT与腹部X线平片在肠梗阻诊断中的应用效果比较

摘　　要：目的评价CT与腹部X线平片在肠梗阻诊断中的应用效果。方法回顾性分析84例肠梗阻患者资料,CT组40例,X线平片组44例,比较两者敏感性、特异性及诊断梗阻原因、类型、部位以及是否存在绞窄情况的准确性。结果CT组敏感性为97.5%,特异性为92.5%;X线组敏感性为77.3%,特异性为84.1%,CT组敏感性显著高于X线组,差异有统计学意义(P<0.05);特异性的比较无统计学意义(P>0.05)。CT组梗阻部位、原因及是否绞窄等情况的确诊率分别为90.0%、87.5%、97.5%;X线组分别为72.7%、31.8%、29.5%,CT组均显著高于X线组,差异有统计学意义(P<0.05)。结论腹部X线平片能够诊断肠梗阻;CT在明确梗阻原因、位置及有无绞窄情况方面更有优势,两种方法可选择或同时应用,为临床治疗提供指导。Objective To observe the application effect of CT and abdominal X - ray plain film in the clinical diagnosis of intesti- nal obstruction. Methods 84 cases of intestinal obstruction were retrospectively analyzed, with 40 cases in the CT group and 44 cases in the X ray plain film group. Comparison between the two groups was made in terms of sensibility, specificity, the obstruction cau- ses, obstructive type, the site of obstruction and the accuracy of determining strangulating intestinal obstruction. Results Sensitivity in the CT group was 97.5% ; specificity was 92. 5% while sensitivity in the X -ray was 77.3% ; specificity was 84. 1%. The sensi- tivity of CT group was significantly higher than that of X - ray group. The difference was statistically significant （ P 〈 0. 05 ） , with no statistically significant difference in specificity （ P 〉 0. 05 ）. The site of obstruction, causes and strangulation diagnosis rate in the CT group were 90. 0% , 87.5% and 97.5% , compared with 72. 7% , 31.8% , and 29. 5% in the X -ray group. These data in the CT group were significantly higher than those in the X - ray group. The difference was statistically significant （ P 〈 0.05 ）. Conclusion While the Abdominal X - ray can examine the intestinal obstruction. CT scanning has advantages in diagnosing the position, causes and the strangulation of obstruction. These two methods can either optional or both applied to provide guidance for clinical treatment.
