摘  要:目的 采用Meta分析方法比较不同BIS条件下对术后早期认知功能障碍(POCD)发生率的影响。方法 检索PubMed、Embase、OVID、CBM、知网、万方、维普等数据库,时间从建库至2014年12月,收集相关不同麻醉深度对术后早期POCD发生率影响的文献,仔细阅读获取的文献摘要和全文,并对其参考文献进行追踪,对纳入的文献进行数据提取及质量评价,并采用RevMan 5.3软件进行Meta分析。结果 共纳入6篇文献,包括521例患者。有4篇文献(255例患者)比较了深麻醉状态(BIS 30~40)与常规麻醉状态(BIS 40~60)术后早期POCD的发生率,深麻醉状态可降低POCD发生率(OR=0.40,95%CI 0.22~0.73,P=0.002)。有3篇文献(216例患者)比较了常规麻醉状态下BIS 40~50与BIS 50~60术后早期POCD的发生率,BIS 40~50并不明显降低POCD发生率(OR=1.11,95%CI 0.24~5.24,P=0.9)。结论 深麻醉状态比常规麻醉状态下POCD发生率低,但由于随机对照试验的数量及质量不足,上述结论尚需开展更大样本及严谨设计的随机对照试验加以论证。Objective To compare the effects of different level of BIS on the incidence of early postoperative cognitive dysfunction (POCD). Methods PubMed, Embase, OVID, CBM, CNKI, Wang- fang data and VIP Database (by December 2014) were searched for randomized controlled trials (RCTs) comparing different depth of anesthesia for their impact on the ineidenee of early POCD. Af- ter data extraction and quality evaluation, RevMan 5.3 software was used for statistical data analysis. Results Six randomized controlled clinical trials involving 521 patients were included in this meta-analysis, 4 (involving a tota/of 255 patients) comparing depth anesthesia (BIS=30-40) with genera/anesthesia (B/S= 40-60), the odds ratio of early POCD incidence were 0. 4(95% CI 0. 22-0. 73,P=0. 002). Other 3 (involving a total of 216 patients) comparing BIS=40-50 with BIS=50-60 in general anesthesia,the odds ratio of early FK)CD incidence were 1.11 (95 % CI 0. 24-5.24, P=0. 9). C.onelmion Compared with general anesthesia, depth anesthesia is associated with a lower incidence of early POCD. But this conclusion needs to be further verified by more well-designed large-scale RCI's.
