摘　　要：目的分析复治涂阳肺结核患者血清中肿瘤坏死因子-α（TNF-α）、白细胞介素-10（IL-10）和白细胞介素-18（IL-18）在治疗前后的变化情况,探讨肺结核细胞免疫机制。方法 2010年1月∽2014年1月随机选取本院住院及门诊确诊为复治涂阳肺结核病患者120例为研究对象,使用利福布丁联合莫西沙星（2HBZEM/6HBE）治疗方案。ABC-ELISA法检测血清TNF-α、IL-10和IL-18治疗前后变化并作比较。结果治疗前、后患者IL-10、TNF-α和IL-18含量差异有统计学意义（P〈0.05）。结论复治涂阳肺结核患者治疗前Thl反应明显占优势,经治疗后血清TNF-α、IL-18、IL-10水平降低,修复Th1/Th2平衡。Objective To analyze the retreatment smear positive pulmonary tuberculosis in patients with tumor necrosis factor-α（TNF-α）,interleukin-10（IL-10）and interleukin-18（IL-18）before and after treatment changes and to explore the mechanism of immune cells in tuberculosis. Methods January 2010 to January 2014 randomly selected hospital inpatient and outpatient diagnosis of retreatment smear positive pulmonary tuberculosis patients treated for 120 cases,using rifabutin joint moxifloxacin（2HBZEM/6HBE）treatment ABG-ELISA was used to detect serum TNF-α,IL-10 and IL-18 treatment and compered chenges. ResultsBefore and after the ABC-ELISA assay of serum TNF-α,IL-10 and IL-18 in the treatment group and compared changes. Results before treatment,serum IL-10,TNF-α and IL-18 were higher（P〈0.05）,the treatment group before and after treatment in patients with IL-10,TNF-α and IL-18 content was significantly sex（P〈0.05）. ConclusionRetreatment smear positive pulmonary tuberculosis patients before treatment was significantly dominant Thl after treatment increased serum TNF-α, IL-18,IL 10-level reducing, repair Th1/Th2 balance.
