摘 要:目的探讨在双源CT上应用"双低"技术对体重指数正常者(BMI≤25 kg/m2)行主动脉CTA检查的可行性。方法应用西门子Flash双源CT对60例体重指数正常者(BMI≤25 kg/m2)行主动脉CTA检查。患者被随机分成两组:A组使用对比剂浓度350 mgI/ml、注射流率4.0 ml/s、用量65 ml、120 kV管电压扫描。B组使用对比剂浓度270 mgI/ml、注射流率4.0 ml/s、用量65 ml、70 kV超低管电压及CARE Dose4D技术扫描,对原始数据进行SAFIRE迭代算法重建。比较两组患者的年龄、BMI,以段为单位测量并比较两组图像主动脉管腔内的CT值、噪声及SNR,对管腔内图像质量进行评估。结果 A、B两组患者在年龄、BMI、SNR无显著差异(P﹥0.05),两组患者在行主动脉CTA检查时接受的辐射剂量分别为(8.34±1.8)mS v和(2.78±0.9)mS v,实验组患者接受的辐射剂量显著降低,差异具有统计学意义(t=50.865,P﹤0.041),两组主动脉管腔内各个节段图像质量均满足诊断要求。结论在西门子Flash双源CT上应用70 kV超低管电压、CARE Dose4D扫描技术并采用SAFIRE迭代算法重建结合低浓度270碘对比剂对体重指数正常(BMI≤25 kg/m2)的患者行主动脉CTA检查可以显著降低患者接受的辐射剂量及碘的摄入量,并能获得可供临床诊断的图像质量。Objective To explore the " double low" technology( BMI≤25 kg /m2) the feasibility of the patients with normal BMI aortic CTA checks on dual source CT. Methods Making aortic CTA scan on 60 patients with chest pain and BMI ≤25 kg / m2 using Siemens Flash dual source CT. All patients were randomly divided into 2 groups. Group A with contrast agent concentration 350 mgI / ml,4. 0 ml / s contrast agent flow rate,dose of contrast agent 65 ml,using 120 kV tube voltage scanning. While group B with contrast agent concentration 270 mgI / ml,4. 0 ml / s contrast agent flow rate,dose of contrast agent 65 ml,using 70 kV ultralow tube voltage and CARE Dose4 D scans. Using SAFIRE iterative algorithm to reconstruct the original data. Comparing two groups of patients with age,BMI,CT number which is measured in the unit of section,noise and signal noise ratio of the picture,eventually the quality of the picture in the unit of section between the two groups. Results There is no significant difference in age,BMI,SNR between the two groups,( P 〉0. 05),while the radiation dose received during the CTA scan in the routine group and experimental group is( 8. 34 + 1. 8)mS v and( 2. 78 + 0. 9) mS v respectively,the difference is statistically significant( t = 50. 865,P 〈 0. 041). Conclusion Using 70 kV ultra low tube voltage and CARE Dose4 D scanning technique combining with low concentration 270 iodine on Siemens Flash dual source CT and SAFIRE iterative algorithm reconstruction on patients with normal BMI when doing the CTA scan will significantly reduce the radiation dose and the intake of iodine,as well as obtaining high quality picture which can be used for clinical evaluation.
