67例重型再生障碍性贫血的临床治疗体会

摘　　要：目的探究与分析重型再生障碍性贫血的临床治疗对策及效果。方法 67例重型再生障碍性贫血患者,采取随机数字表法分为对照组34例,试验组33例。两组均给予环孢菌素A联合雄激素治疗,试验组在此基础上加用中剂量环磷酰胺治疗,对比两组临床疗效及毒副反应。结果对照组总有效率为61.76%,试验组总有效率为84.85%,组间差异具有统计学意义(P<0.05)。对照组与试验组脱发、感染、肝肾功能损害、胃肠道反应及出血性膀胱炎的发生率相比,差异均无统计学意义(P>0.05)。结论于环孢菌素A联合雄激素治疗的基础上加用中剂量环磷酰胺治疗重型再生障碍性贫血的疗效显著,且不增加不良反应,值得推广。Objective To investigate and analyze clinical treatment measures and effects of severe aplastic anemia.Methods A total of 67 patients with severe aplastic anemia were divided by random number table into control group with 34 cases and experimental group with 33 cases. Both groups received cyclosporine A combined with androgen for treatment, and the experimental group received additional cyclophosphamide by moderate dose. Clinical effects, toxic and side effects of the two groups were compared.Results Total effective rate was 61.67% in the control group, and 84.85% in the experimental group. The difference between the two groups had statistical significance （P〈0.05）. There were no statistically significant differences of incidences of alopecia, infection, liver and kidney function injury, gastrointestinal reaction, and hemorrhagic cystitis between the two groups （P〉0.05）.Conclusion Additional implement of cyclophosphamide by moderate dose to combination of cyclosporine A and androgen provides remarkable effect in treating severe aplastic anemia, without increased adverse reactions. This method is worthy of promoting.
