三维标测系统指引下消融特发性左前分支起源室性期前收缩2例

摘　　要：左室特发性室速多数起源于左后分支区域,心电图表现为右束支阻滞伴电轴左偏,其发生与分支参与折返有关。而少数表现为电轴右偏,起源则位于左心室游离壁或主动脉瓣窦下方的左心室流出道部位,该部位为左前分支分布的区域。在左室特发性室性心律失常中,左前分支室速或室早的发生率不到8%,是临床上少见的心律失常。本文报道2例应用Ensite 3000三维标测系统指引下行左前分支起源室性期前收缩的射频消融治疗。The ventricular premature beat which originated from left anterior fascicular is a rare type of pre- mature ventricular complexes that are observed in the clinic. Its electrophysiological mechanism is reentrant cir- cuit. We can map the Purkinje potential as the target site to perform ablation, until the ECG shows left anterior fascicular blocking. In this article, we report 2 cases of anterior fascicular ventricular premature beat, which re- ceived successful ablation under the guidance of the three-dimensional mapping system. This paper analyzes the process of ablation and summarizes the procedural experiences.
