摘  要:目的:采用磁共振弥散张量成像(DTI)及纤维束示踪(FT)技术,结合传统神经导航,提高对肿瘤周边传导束的定位精度,引导医生在最大程度切除肿瘤的同时尽可能降低对传导束的损伤。方法:将DTI及FT功能整合在excelim-04神经外科手术导航系统上,对DTI影像进行处理,并与解剖学影像融合,再利用纤维束示踪技术,重建三维传导束,最后按照导航指示的肿瘤与传导束的边界进行手术,并根据术中判断与术后随访结果验证效果。结果:导航手术对肿瘤及周围传导束的边界定位效果良好,在提高肿瘤全切率的同时降低了对传导束的损伤,术后随访患者生活正常,无严重并发症。结论:excelim-04神经外科手术导航系统的DTI和FT技术提高了涉及脑功能区和白质肿瘤手术的安全性。Objective: To Improve the positioning accuracy of the tracts around the tumor and help doctors reduce the damage to the tracts when maximizing the removal of the tumor with the adoption of the diffusion tensor Imaging(DTI) and fiber tracing(FT) technology combining the traditional neuronavigation.Method: Process the DTI images and fuse them with the anatomical images. Reconstruct the three-dimensional tracts with the FT technology and perform the operation folowing the navigation on the boundary of the tumor and the tracts. then verify the effects according to the intraoperative results and postoperative folow-up.Results: The effect of the navigation on the boundary of the tumor and the tracts is good. The navigation helped doctors reduce the damage to the tracts when maximizing the removal of the tumor. The patients were folowed up with normal life without serious complications.Conclusions: The safety of the operation of the tumor which involved the function area of brain and the white mater has been improved with the adoption of the DTI and FT technology by the image guided system of excelim-04 in neurosurgery.
