摘 要:目的 探讨乙型肝炎病毒表面抗原(hepatitis B surface antigen,HBsAg)阳性母亲新生儿外周血白细胞介素-10(interleukin-10,IL-10)和白细胞介素-12(interleukin-12,IL-12)水平与其接种乙肝疫苗无、弱应答的关系。方法 选择98对HBsAg阳性母亲及其足月分娩新生儿,采用荧光定量聚合酶链反应(fluorescent quantitation poly-merase chain reaction,FQ-PCR)和化学发光免疫试验(chemiluminescence immunoassay,CLIA)分别检测母儿乙型肝炎病毒脱氧核糖核酸(hepatitis B virus deoxyribonucleic acid,HBVDNA)和HBV血清学标志物,酶联免疫吸附试验(en-zyme-linked immunosorbant assay,ELISA)检测新生儿外周血IL-10、IL-12水平。新生儿全程接种乙肝疫苗随访至12月龄,检测血清抗-HBs水平。结果 乙肝疫苗无/弱应答率为32.65%。单因素分析,新生儿外周血IL-10和IL-12水平与乙肝疫苗免疫应答有关(均有P〈0.05)。非条件Logistic回归模型调整母亲HBeAg、母亲HBVDNA、新生儿乙型肝炎e抗原(hepatitis Beantigen,HBeAg)等因素,新生儿外周血IL-10水平〈174.05pg/ml者易发生乙肝疫苗免疫无或弱应答(OR=8.35,95%CI:1.83-38.14)。分级分析,乙肝疫苗无/弱应答率随IL-10水平增加呈下降趋势(2趋势=4.41,P=0.036)。结论 HBsAg阳性母亲婴儿乙肝疫苗免疫无或弱应答可能与其新生儿期外周血IL-10水平低下有关。Objective To investigate the relationship between the level of interleukin-10( IL-10) and interleukin-12( IL-12) in neonatal peripheral blood and non-/ low-responsiveness to hepatitis B vaccine of infants born to HBs Ag positive mothers. Methods Ninety-eight HBs Ag positive mothers and their full-term neonates were enrolled. Maternal and neonatal hepatitis B virus( HBV) DNA and HBV serologic markers were measured using chemiluminescence immunoassay( CLIA) and fluorescence quantitative polymerase chain reaction( FQ-PCR),respectively. The level of IL-10 and IL-12 in neonatal peripheral blood were determined by enzyme-linked immunosorbant assay( ELISA). These neonates were followed up at the age of one year,and the anti-HB titer of these infants were determined by CLIA. Results The rate of non-/ lowresponsiveness was 32. 65%. In univariate analysis,the level of IL-10 and IL-12 in neonatal peripheral blood was significantly associated with responsiveness to hepatitis B vaccine( all P〈0. 05). Maternal HBe Ag,maternal HBV DNA,neonatal hepatitis B e antigen( HBe Ag) and other factors were adjusted by unconditional logistic regression model. The result revealed that when the level of IL-10 in neonatal peripheral blood was less than 174. 05 pg / ml,the infants were more likely to be non-/ low-responsive to hepatitis B vaccine( OR = 8. 35,95% CI: 1. 83-38. 14). Classification analysis showed that the rate of non-/ low-responsive was negatively associated with the level of IL-10(χ2trend= 4. 41,P = 0. 036). Conclusions Infants born to HBs Ag positive mothers with lower level of IL-10 in neonatal peripheral blood were more likely to be non-/low-responsive to hepatitis B vaccine.
