摘  要:分析髋关节假体置换术后松动并股骨骨折的翻修方法。方法:我院2010年1月至2013年12月共收治18例髋关节假体置换术后出现假体松动和股骨骨折的患者,翻修距初次假体置换术时间为6~90个月。18例患者中有6例为无茵性的假体松动,12例患者股骨骨折,其中骨折患者根据Vancouver标准划分A型患者1例,B1型患者5例,B2型患者3例,B3型患者1例。C型患者2例。不同类型患者接受不同的翻修治疗,疗程结束后进行6~36个月的随访。结果:患者翻修手术后切口都是Ⅰ期愈合,随访期间没有复发股骨骨折和假体松动,也没有出现感染。治疗前后的Harris评分和VAS疼痛评分差异明显,具有统计学意义(P〈0.05)。结论:髋关节假体置换术后出现假体松动或股骨骨折,需要全面地评估患者情况,制定针对性翻修方案治疗,并在术后进行随访防止并发痘的出现。Objective : Analysis of loose and femur fracture after hip prosthesis replacement overhaul method.Methods : Our hospital between January 2010 and December 2010, a total of 18 cases of prostheses loosening after hip prosthesis replacement and patients with femoral fracture, renovated from primary prosthetic replacement for 6 - 90 months.Of 18 cases with 6 cases for prosthetic aseptic loosening, 12 cases with femoral fracture, the fracture patients according to the division standard of more than 1 patients with type A, B1 type 5 cases patients, 3 patients with B2 type, B3 type 1 patients, 2 patients with C.Different types of patients treated with different renovation, after treatment for 6 ~ 36 months of foUow-up.Results = Patients are I healing of incision after revision surgery, no recurrence during the follow-up period femoral fractures and prosthesis loosening, and no infection.Harris scores before and after treatment and VAS pain score difference to be markedly, with statistical significance (P 〈 0.05) .Conclusion = After hip prosthesis replacement prosthesis loosening or femur fracture, need to comprehensively assess the clinical situation, treatment, targeted overhaul plan and follow-up after surgery to prevent the occurrence of complications.
