摘 要:目的探讨调节性T细胞检测在浆细胞性乳腺炎(PCM)诊断中的临床价值。方法将2008年5月至2013年5月华润武钢总医院收治的62例PCM患者按照病程分为急性期(18例)、亚急性期(31例)和慢性期(13例);另选择30例乳腺癌患者和30例健康女性作为乳腺癌组和健康对照组;比较各组外周血CD4+CD25+CD127-调节性T细胞水平变化。结果健康对照组、乳腺癌组、PCM组外周血CD4+CD25+CD127-T细胞、CD4+CD25+Fox P3+T细胞百分率、转化生长因子β(TGF-β)分别为(4.0±1.8)%、(4.4±1.4)%、(87±16)mg/L;(3.0±0.5)%、(2.4±1.1)%、(53±19)mg/L;(5.8±2.2)%、(5.5±1.0)%、(385±37)mg/L。PCM组显著低于健康对照组(P<0.05);乳腺癌组患者各指标显著高于对照组(P<0.05)。急性期PCM组CD4+CD25+CD127-T细胞、CD4+CD25+Fox P3+T细胞、TGF-β为(2.8±1.2)%、(1.6±0.7)%、(35±11)mg/L,亚急性期分别为(3.1±1.2)%、(2.4±1.0)%、(56±14)mg/L,慢性期为(3.5±1.2)%、(3.3±1.3)%、(70±22)mg/L,急性期显著低于亚急性期和慢性期,亚急性期显著低于慢性期(P<0.05)。相关性分析显示,CD4+CD25+CD127-T细胞与CD4+CD25+Fox P3+T细胞、TGF-β呈正相关(r=0.589,0.724,P<0.05),与C反应蛋白、红细胞沉降率、Ig A、Ig G、Ig M、C3、C4、血小板计数、血红蛋白无明显相关性(P>0.05)。结论 PCM患者外周血CD4+CD25+CD127-T细胞显著降低,可以作为筛查PCM的指标之一。Objective To explore the clinical value of regulatory T cell detection diagnosis in patients with plasma cell mastitis( PCM). Methods A total of 62 cases of PCM treated in Huarun Group Wuhan Steel General Hospital during May 2008 and May 2013 according to the disease course were divided into18 cases of acute phase,31 cases of subacute phase and 13 cases of chronic phase; another 30 cases of breast cancer patients and 30 healthy women were selected as breast cancer group and healthy control group; the peripheral blood levels of CD4+CD25+CD127-regulatory T cells of each group were compared.
