摘 要:目的研究微创手术对阑尾手术患者切口感染的影响。方法回顾性分析治疗的急性阑尾炎患者319例,将采用微创手术进行治疗的158例患者,作为微创手术组;采用开腹手术进行治疗的161例患者,作为开腹组。采用SPSS17.0软件进行统计学分析,性别、病理类型、切口感染等计数资料的比较采用χ2检验;年龄、术中出血量、手术时间、住院时间等计量数据呈正态分布采用t检验,P<0.05则差异具有统计学意义。结果两组患者在手术时间上无显著差异(P>0.05);与开腹组[(23.4±6.9)ml、(7.5±2.2)d]相比,微创手术组的术中出血量(12.6±3.3)ml显著减少、住院时间(4.2±1.3)d显著缩短;与开腹组9.6%(7/73)相比,微创手术组患者化脓性的切口感染率1.4%(1/71)显著降低(χ2=4.591,P<0.05);两组差异均有统计学意义(P<0.05)。两组患者在单纯性、坏疽性两个病理类型方面无明显差异(P>0.05)。结论微创手术能够显著降低阑尾手术患者的切口感染率,减少患者的术中出血量,缩短患者的住院时间。Objective To study the impact of minimally invasive surgery on incision infection of patients who underwent appendectomy. Methods The clinical data of 319 patients with acute appendicitis in our hospital were analyzed retrospectively.Minimally invasive operation was performed in 158 patients (minimally invasive operation group) and open operation in 161 patients (open operation group).Statistical analysis was performed using SPSS17.0 software.The data on gender, pathological type, and incision infection were assessed by the Chi-square test.The data of normal distribution of age, intraoperative bleeding volume, operation time, hospitalization time were measured by Student's t test.A P value less than 0.05 was considered statistically significant. Results There was no significant difference in operation time between the two groups (P〉0.05). The volume of bleeding during the operation was (12.6 ±3.3) ml, which was reduced more significantly in the minimally invasive operation group than in the open operation group (23.4 ±6.9) ml.The hospitalization time in the minimally invasive operation group was decreased more significantly than in the open operation group.The infection rate of purulent wound was decreased significantly to 1.4%(1/71) in the minimally invasive operation group (χ^2=4.591, P〈0.05).(P〈0.05).There was no significant difference in pathological types or simple and gangrenous appendicitis between the two groups ( P 〉0.05). Conclusion The minimally invasive operation could significantly reduce the rate of incision infection in patients with appendix operation by reducing the bleeding volume of patients and decreasing the hospitalization time.
