高龄患者环状混合痔并发慢性病行PPH术围术期处理

摘　　要：目的探讨高龄患者环状混合痔并发慢性病手术治疗时应注意的问题。方法对220例环状混合痔手术治疗进行回顾性分析,其中并发慢性病者119例(冠心病39例,高血压病55例,糖尿病25例)。结果手术时行腰麻或硬膜外阻滞者93例,局麻6例,全麻20例。均行吻合器痔上黏膜环切术(PPH术),随访3年,无肛门狭窄及痔复发。结论伴有高血压病、冠心病或糖尿病者,术前处理是围术期处理的关键。Objective To investigate the surgical treatment of elderly patients with circumferential mixed hemorrhoids concurrent chronic diseases. Methods A retrospective analysis of 220 cases of circumferential mixed hemorrhoids surgery was performed, of which 119 cases were concurrent chronic diseases （39 cases of coronary heart disease, 55 cases of hypertension, 25 cases of diabetes）. Results During surgery, 93 ca- ses were given spinal or epidural anesthesia, local anesthesia 6 cases, general anesthesia, 20 cases. PPH surgery was performed, with 3 - year follow- up, no anal stenosis and hemorrhoids recurrence. Conclusion For the patients with hypertension, coronary heart disease Or diabetes, preoperative treatment is the key to perioperative management.
