摘 要:目的观察肺动脉排气对气体栓塞诱导猪肺功能损伤的保护作用及其机制。方法制备气体栓塞诱导猪肺功能损伤模型。将实验猪随机分为3组,即假手术组(A组)、肺动脉气栓组(B组)、肺动脉气栓+肺动脉排气组(C组)。各组经过相应处理后,分别在开胸前(T1)与肺动脉排气后(T2)检测肺功能,即肺动脉压力指数、氧和指数以及肺静脉血中心粒细胞计数。用Elisa法检测各组实验猪动脉血血浆中MDA、TNF-α、IL-6、IL-10的表达水平。结果与A组相比,B组肺功能明显下降(P<0.01),同时MDA、TNF-α、IL-6、IL-10的含量明显增多(P<0.01)。而与B组相比,C组肺功能指标明显改善(P<0.05),MDA、TNF-α、IL-6、IL-10的含量显著下降(P<0.05)。结论肺动脉排气对肺动脉气栓诱导实验猪肺功能损伤具有显著的保护作用,该作用可能与减少肺部血管的炎症损伤相关。Objective To investigate the protective effects of pulmonary artery exsufflation against lung damage induced by aeroembolism in swine and its mechanisms. Methods The lung damage was induced by air embolism in 24 anesthetized, open-- chest pigs. The groups of swine were divided into A (sham) group, B (aeroembolism) group,and C(aeroembolism + exsuffla- tion treatment) group. Lung function index,and neutrophil were detected before and after operation. The expression levels of MDA,TNF-α,IL-6,IL-10 were also observed by Elisa. Results Comparing with A group, lung function index of B group de- creased significantly(P〈0.01 ) ,and MDA,TNF-α,IL-6, IL-10 and numeration of neutrophil increased significantly( P〈0.01). Compared with B group lung function index of C group improved significantly(P〈0.05 ), and inflammatory factors and cells decreased significantly (P〈0.05 ). Conclusion Pulmonary artery exsufflation can protect the lung function and enhance the production of NO by inhibiting inflammatory reaction against lung damage induced by aeroembolism.
