摘  要:目的探讨燃煤电厂作业人员职业应激与糖脂代谢水平的关系。方法采用整群抽样方法,选择8家燃煤电厂525名作业人员为研究对象,采用《职业紧张量表》调查作业人员的职业应激水平,采集作业人员肘静脉血测定血糖(GLU)和血脂水平。结果 GLU和低密度脂蛋白(LDL)水平均分别与职业任务分量表(ORQ)、紧张反应分量表得分呈正相关(P<0.01);GLU和TG水平分别与应对资源分量表得分呈负相关(P<0.01)。525名作业人员以及其中的238名管理人员中,其GLU和LDL水平均呈现随着职业应激水平增高而增高的剂量-效应关系(P<0.05);相较于职业应激低和中水平组,职业应激高水平组的总胆固醇和甘油三酯(TG)水平均升高(P<0.05),高密度脂蛋白水平下降(P<0.05)。287名生产人员中,其GLU水平有随着职业应激水平增高而增高的剂量-效应关系(P<0.05),职业应激高水平组LDL和TG水平均分别高于职业应激低、中水平组(P<0.05)。结论燃煤电厂作业人员职业应激与糖脂代谢水平之间存在相关性;职业应激水平越高,对糖脂代谢影响越明显。Objective To explore the correlation between occupational stress and glycolipid metabolism level in coal-fired power plant workers. Methods By using cluster sampling method, 525 workers from 8 coal-fired power plants were selected as observation subjects, the Occupational Stress Inventory-revised (OSI-R) scale was used to investigate the occupational stress level of workers, the elbow vein blood were collected to detect the blood glucose (GLU) and blood lipid levels. Results The levels of GLU and low density lipoprotein (LDL) were positively correlated with the scores of Occupation Role Questionnaire (ORQ) and Personal Strain Questionnaire of OSI-R ( P 〈 0. Ol ). The levels of GLU and triglyceride (TG) were negatively correlated with the scores of Personal Resources Questionnaire of OSI-R (P 〈 0.01 ). Among the 525 observation subjects and the 238 management staffs of them, the levels of GLU and LDL all increased with occupational stress levels increased in dose-effect relationship respectively ( P 〈 0.05 ) ; compared with the groups of low and middle occupational stress levels, the total cholesterol and TG were all increased ( P 〈 0.05 ), but the high density lipoprotein level decreased (P 〈 0.05 ). Among the 287 production personnel, the GLU level increased with occupational stress levels increased in dose-effect relationship respectively (P 〈 0.05), and the levels of LDL and TG in group of high occupational stress level were higher than those in groups of middle and low occupational stress level ( P 〈 0.05 ). Conclusion The occupational stress was correlated to glycolipid metabolism level in coal-fired power plant workers; higher occupational stress level could cause more obvious effect on the glycolipid metabolism.
