血栓弹力图与血小板指标在脑梗死患者中的相关性研究

摘　　要：目的探讨血栓弹力图(TEG)与血小板相关指标的关系,并分析脑梗死患者的TEG与血小板指标的变化。方法收集本院脑梗死患者73例作为实验组,另检测非脑梗死患者58例作为对照组,采用方差分析比较脑梗死患者与对照组的TEG及血小板指标的变化,并采用Pearson相关分析脑梗死患者TEG及血小板指标的相关性。结果脑梗死患者73例TEG指标的R、K值低于对照组(P<0.05),Angle角、CI高于对照组(P<0.05),血小板指标中平均血小板体积、大血小板比率和血小板体积分布宽度高于对照组(P<0.05)。脑梗死患者TEG指标中R值与平均血小板体积、大血小板比率之间呈负相关(P<0.05),MA值与血小板体积呈正相关(P<0.05),脑梗死患者其他TEG指标与血小板指标之间相关系数无统计学意义(P>0.05)。结论 TEG可用于急性脑梗死患者血液凝固性异常的诊断,TEG指导与监测药物治疗和评估患者预后应用前景值得进一步探讨。Objective To investigate the relationship between TEG and platelet-indices and analyze the changes of cerebral infarction in patients with TEG and platelet-indices. Methods 73 patients with cerebral infarction were collected as the experimental group, and the other 58 patients as the control group. Analysis of variance was used to investigate TEG and platelet index comparison with the control group of patients with cerebral infarction and to explore the relationship between TEG and platelet-indices with Pearson correlation analysis. Re- suits TEG indicators in the experimental group had lower R and K values than the control group （P 〈 0. 05）. Angle angle and CI were higher than control group （ P 〈 0.05）. A comparison on the mean platelet volume platelet index, the ratio of large platelets and platelet volume distribution with the control group also showed statistical significance （ P 〈 0. 05 ）. TEG index in patients with cerebral infarction and R values mean platelet volume, platelet ratio showed a large negative correlation （ P 〈 0. 05）. MA and platelet hematocrit Value were positively correlated （ P 〈 0. 05 ） and there was no significant correlation between other TEG indicators and platelet index （P 〉 0. 05）. Conclusion TEG can be used in patients with acute cerebral infarction diagnosis of abnormal blood clotting. Further TEG guidance, monitoring of drug therapy and evaluation of prognosis are worthy of further discussion.
