一种定量PCR试剂盒通过血液诊断HCMV感染效能的Meta分析

摘　　要：目的通过Meta分析,评价达安荧光定量PCR试剂盒通过血液诊断HCMV感染的效能,为HCMV血筛方法的研发提供依据。方法计算机检索中文科技期刊全文数据库、中国期刊全文数据库、万方数字化期刊、Pub Med等数据库,检索通过达安荧光定量PCR试剂盒血液诊断HCMV感染的文献,按照纳入和排除标准筛选文献,提取纳入文献的数据,采用Meta-Disc软件进行Meta分析,检验异质性,根据异质性结果选择相应的效应模型,对纳入文献予以加权定量合并,计算汇总敏感度、特异度、阳性似然比、阴性似然比和诊断优势比及其95%CI,绘制汇总受试者工作特征(SROC)曲线,并计算曲线下面积(AUC)。结果共检出相关文献829篇,按照文献纳入标准,最终纳入5篇文献,汇总敏感度为0.94,95%CI(0.89-0.97)、汇总特异度为0.92,95%CI(0.89-0.85)、汇总阳性似然比为11.37,95%CI(7.4-17.46)、汇总阴性似然比为0.07,95%CI(0.04-0.13)、汇总诊断比数比为169.90,95%CI(74.00-390.06),SROC AUC为0.98。结论达安荧光PCR定量试剂盒通过血液诊断HCMV感染具有较高的灵敏度、特异度及稳定性。Objective To evaluate the diagnostic value of FQ-PCR kit of Da＇an company for the HCMV infection by blood samples, with HCMV pp65 antigenemia assay as the reference standard, with the purpose to contribute to the development of blood screening method of HCMV. Methods PubMed, EMBASE, CNKI, VIP and Wanfang were searched tO find all diagnostic tests about PCR for HCMV. After collecting studies according to inclusion and exclusion criteria, data on study background, design information and diagnostic parameters were extracted. Meta-disc software was used to handle data from the included studies and to exmnine heterogeneity. The effect-model was selected according to outcomes of heterogeneity. All included studies were combined with weighted quantity, sensitivity, specificity, positive likelihood ratio （PLR）, negative likelihood ratio（ NLR）, diagnostic odds ratio（DOR）. Their 95% CI were calculated, and an SROC curve was drawn. Re- suits Potential relevant 829 studies were collected. According to eligibility criteria, 5 studies compared with HCMV pp65 antigenemia assay were included and outcomes were as followed： pooled sensitivity 0. 94, 95% CI（0. 89 -0. 97 ）, pooled specificity 0. 92, 95 % C I （0. 89 -0. 85 ）, PLR11.37, 95% CI （7. 4 - 17.46）, NLR 0. 07, 95 % C I （0. 04 -0. 13 ）, DOR 169. 90, 95%CI （74. 00 -390. 06） and SROC AUC 0. 98. Conclusion The diagnostic value of FQ-PCR kit of Da＇n company for the HCMV infection through blood samples is high and FQ-PCR test can be regarded as the best candidate to be developed for the blood screening method of HCMV.
