摘 要:目的:探讨妊娠期间合并糖尿病接受治疗的孕妇、不接受治疗的孕妇以及妊娠合并轻度糖尿病的孕妇,这三者间胰岛素、瘦素、脐血糖以及脂联素水平存在的差异。方法:选取本院收治的120例妊娠合并糖尿病孕妇,随机分为3组,即妊娠期糖尿病组、妊娠期糖尿病对照组、妊娠期糖尿病治疗组,每组40例,3组孕妇均给予常规孕妇管理,在此基础上,给予治疗组进行血糖监测、以及饮食控制等治疗,根据患者的需要,可给予胰岛素治疗。然后对3组进行胰岛素、瘦素、脐血糖以及脂联素的检测。结果:糖尿病组瘦素水平明显高于对照组及治疗组,脂联素及水平脂联素/瘦素比值明显低于对照组及治疗组,差异具有统计学意义(P<0.05);治疗组脂联素水平明显低于对照组,差异具有统计学意义(P<0.05);3组孕妇空腹血糖比较,差异无统计学意义(P>0.05);治疗组及对照组早餐、午餐及晚餐后血糖均明显低于糖尿病组,差异具有统计学意义(P<0.05);对照组早餐、午餐及晚餐后血糖均明显低于治疗组,差异具有统计学意义(P<0.05)。结论:妊娠合并糖尿病对婴儿脂肪代谢造成一定的影响,即可改变瘦素、血糖以及脂联素水平,对患者进行相关的治疗,能够将有效控制这种改变,提高临床疗效,值得推广。Objective:To explore the differences in insulin,leptin,umbilical blood sugar,and adiponectin levels among the pregnant women with gestational diabetes treatment,without gestational diabetes treatment,and the pregnant women with mild gestational diabetes.Methods:A total of 120 pregnant women with gestational diabetes who were admitted in our hospital from May,2011 to July,2012 were included in the study and randomized into three groups,ie.gestational diabetes group,gestational diabetes control group,and gestational diabetes treatment group,with 40 cases in each group.The pregnant women in the tree groups were given routine care.On the basis of routine care,the pregnant women in the treatment group were given blood sugar monitoring and diet control,and insulin treatment was provided if necessary.Then the insulin,leptin,umbilical blood sugar,and adiponectin levels in the three groups were detected.Results:The leptin level in the diabetes group was significantly higher than that in the control group and the treatment group(P〈0.05),while the adiponectin level and adiponectin/leptin ratio were significantly lower than those in the control group and the treatment group(P〈0.05).The adiponectin level in the treatment group was significantly lower than that in the control group(P〈0.05).The difference of FBG among the three groups was not statistically significant(P〈0.05).The blood sugar levels after breakfast,lunch,and dinner in the treatment group and control group were significantly lower than those in the diabetes group(P〉0.05).The blood sugar levels after breakfast,lunch,and dinner in the control group were significantly lower than those in the treatment group(P〈0.05).Conclusions:Gestational diabetes can give rise to a certain effect on the fetal fat metabolism by altering the leptin,blood sugar,and adiponectin levels.The related treatment on the patients can effectively control this alteration,enhance the clinical efficacy.It deserves a wide recommendation.
