零切迹椎间融合内固定系统治疗脊髓型颈椎病患者的护理

摘　　要：对56例行零切迹椎间融合内固定系统治疗的脊髓型颈椎病患者,术前给予健康教育和心理护理、术后早期给予颈部及四肢康复指导。术后切口内血肿致呼吸困难2例、神经症状加重1例、脑脊液漏4例,经护理干预后均顺利恢复;呛咳吞咽不适3例,术后2周内消失,2周后无1例存在吞咽不适或咽部异物感。未发生松动、移位、断裂等内固定相关并发症,无护理相关并发症。术后12个月随访,神经功能改善率为69.37%,视觉模拟评分(visual analogue scale,VAS)由术前的(4.45±1.66)分降为(1.03±0.79)分。认为有针对性的围手术期护理干预,可促进脊髓型颈椎病患者术后功能的恢复,降低并发症发生率。For 56 patients with cervical spondylotic myelopathy treated by zero profile anterior cervical interbody fusion system, health education and psychological nursing were conducted preoperatively, and neck and limbs reha- bilitation guidance were provided postoperatively. The incision hematoma causing dyspnea in 2 cases,postoperative neurologic deterioration in 1 case, 4 cases of cerebrospinal fluid leakage, all of which recovered after nursing inter- vention; cough swallowing discomfort in 3 cases, symptoms disappeared in 2 weeks after operation. There was no loosening, displacement, fracture and other internal fixation related complications, and no nursing related complica- tions. Follow-up of 12 months after surgery,the neurological function was improved by 69.37%,and VAS score was deceased from 4.45±1.66 to 1.03±0.79. Targeted perioperative nursing intervention is considered to be able to pro- mote function of patients with cervical myelopathy, and to reduce the rate of complications.
