摘 要:目的探讨膀胱温度与腋窝温度在不同体温分度上的差异性与相关性,为测温导尿管的临床应用提供科学依据。方法对2014年9—10月就诊于我院ICU的42例重症患者,采用同期自身对照方法,分别使用测温导尿管测量膀胱温度和水银体温计测量腋温,比较两种测量结果间的差异性与相关性。结果共测得252组体温数据,从两种体温值差异性上看:总体体温比较,两种体温值间差异无统计学意义(38.44±1.11)VS(37.87±1.10),(P〉0.05);不同体温分度比较,腋温在36.1~37℃和37.1~38℃时,两种体温值间差异无统计学意义(P〈0.05);而腋温在35~36℃、38.1~39℃及39℃以上时,两种体温值间存在不同程度的差异,差值分别为(0.44±0.24)、(0.56±0.12)和(0.56±0.12),且差异有统计学意义(P〈0.05)。从两种体温值相关性上看:总体体温比较,两种体温值相关性较好,相关系数r=0.912,P〈0.05;其中,腋温在36.1~37℃和37.1~38℃时,两种体温值相关系数r达到0.932和0.921(P〈0.05),而腋温在35~36℃、38.1~39℃及39℃以上时,两种体温值间相关性存在差异,相关系数r分别为0.732、0.802和0.725(P〈0.05)。结论膀胱温度与腋窝温度之间总体差异较小,相关性较好,而腋温在35~36℃、38.1~39℃及39.1℃以上时,两种体温值之间的差异性和相关性还有待进一步探讨。Objective To explore the difference and correlation between urinary bladder temperature and axillary temperature for ICU patients. Method 42 cases of critically ill patients in ICU from September 2014 to October 2014 in our hospital was selected for the study using self control method at the same period. Bladder temperature was measured by urinary temperature catheter, and axillary temperature was measured by mercury thermometer. The difference and correlation between the different measuring results was compared. Result 252 data was analyzed, There was no significant difference between the urinary bladder temperature and axillary temperature (38.24±1.11 vs(37.77±1. 10) for overall comparison(P〉0.05). Compare different temperature groups, there was no significant difference between the two difference temperature when axillary temperature was at 36. 1-37 ℃ and 37.1-38 ℃ (P〉0.05). There are some of difference between the two temperature when axillary temperature was at 35-36 ℃, 38.1-39 ℃ and〉39 ℃,The differences were(0. 445=0.24),(0.56±0.12) and (0.56±0. 12), (P〈0.05). The different temperature was correlated compare different temperature groups (correlation coefficient r = 0. 912 (P〈0.05). The correlation coefficient reached 0. 932 and 0. 921 while axillary temperature was at 35-36 ℃ ,38.1 39 ℃ and -39 ℃. The correlation coefficient r were 0. 732, 0. 802 and 0. 725 compared two groups with different temperature (P〈 0. 05). Conclusion Compared with urinary bladder temperature and axillary temperature, the difference were small and better correlated. Further study was needed for exploring the difference and correlation when axillary temperature was at 35-36 ℃, 38.1-39 ℃ and 〉39 ℃.
