脐血IgE及母血IgE与婴儿过敏性疾病的相关性研究

摘　　要：目的 研究脐血IgE及母血IgE与婴儿过敏性疾病的关系,探讨母亲过敏史与婴儿过敏性疾病的相关性。方法 选取2011年3月-2012年2月在佛山市禅城区中心医院产检的孕妇共40例,对孕妇进行过敏性疾病问卷调查,孕期行母血IgE、分娩后行脐血IgE检测。对出生后的婴儿进行为期6个月的随访,观察婴儿有无出现过敏性疾病。结果 脐血IgE与母血IgE水平差异有统计学意义（P〈0.05）。母亲有过敏史及无过敏史组的母血IgE水平差异无统计学意义（P〉0.05）,脐血IgE水平差异有统计学意义（P〈0.05）,婴儿过敏性疾病发生率差异无统计学意义（P〉0.05）。新生儿脐血IgE水平正常组（IgE〈350 U/L）与升高组（IgE≥350 U/L）的过敏性疾病发生率差异有统计学意义（P〈0.05）。结论 母亲有过敏史更易提高胎儿的过敏风险,孕期监测母血IgE及脐血IgE对预测新生儿过敏性疾病有很大的临床价值,脐血IgE对于新生儿过敏性疾病的预测价值高于母血IgE。Objective To investigate the relationship between cord blood Ig E,maternal blood Ig E and infant allergic disease to discuss the correlation between mother′ s allergic history and infant allergic disease. Methods 40 pregnant women given antenatal care in Chancheng District Central Hospital of Foshan City from March 2011 to Februar 2012 were selected,and questionnaire survey of allergic disease was conducted for pregnant women respectively.Detection of maternal blood Ig E and cord blood Ig E was conducted during pregnancy and after delivery.Infants after birth were followed-up for 6 months,whether there was an allergic disease in infant was observed. Results There was a statistical difference of cord blood Ig E and maternal blood Ig E level（P〈0.05）.There was no statistical difference of maternal blood Ig E level（P〈0.05）,and there was a statistical difference of cord blood Ig E level（P〉0.05）,and there was no statistical difference of the incidence rate of infant allergic disease（P〉0.05） between mother having history of allergy group and mother having no history of allergy group.There was a statistical difference of the incidence rate of allergic disease between neonatal normal cord blood Ig E level group（Ig E 〈350 U/L） and neonatal elevated cord blood Ig E level group（Ig E≥350 U/L）（P〈0.05）. Conclusion Mother with a history of allergy is more likely to raise allergy risk of fetus,maternal blood Ig E and cord blood Ig E monitored during has a great clinical value,and predictive value of cord blood Ig E is higher than maternal blood Ig E for neonatal allergic disease.
