吗替麦考酚酯联合恩替卡韦治疗乙型肝炎相关性肾炎的效果分析

摘　　要：目的探讨吗替麦考酚酯联合恩替卡韦治疗乙型肝炎相关性肾炎（HBV-GN）的临床效果。方法选择2011年1月-2014年6月我院收治的58例HBV-GN患者,按照随机数字表法分为对照组和研究组,各29例。对照组给予恩替卡韦治疗,研究组患者给予吗替麦考酚酯联合恩替卡韦治疗,疗程均为6个月。比较两组患者治疗前后血肌酐、24 h尿蛋白定量、ALT和Alb变化及临床效果。结果研究组患者总有效率明显高于对照组,差异有统计学意义（93.1%vs 72.4%,χ^2=5.584,P=0.041）。治疗后,两组患者24 h尿蛋白定量和ALT均明显降低,Alb明显升高,且研究组患者变化更为明显,差异有统计学意义（P〈0.05）。结论吗替麦考酚酯联合恩替卡韦能明显改善HBV-GN患者的肝肾功能,提高治疗效果,值得临床推广应用。Objective To observe the clinical curative effect of mycophenolate mofetil combined with entecavir in the treatment of patients with hepatitis B virus associated nephritis（HBV-GN）. Methods 58 patients with HBV-GN from January 2011 to June 2014 in our hospital were selected and were divided randomly and voluntarily into control group（n=29） and research group（n=29）.The patients of control group were given entecavir therapy while the research group were treated by mycophenolate mofetil combined with entecavir,the course was six months.The serum creatinine,24 hours proteinuria,ALT and Alb of before and after treatment and clinical effects between two groups was compared respectively. Results The total effective rate of research group was obviously higher than that of control group,with statistical difference（93.1% vs 72.4%,χ^2=5.584,P=0.041）.The 24 hours proteinuria and ALT of after treatment of all patients reduced while the Alb increased obviously,and the observation group showed more,with statistical difference（P〈0.05）.Conclusion Treatment of mycophenolate mofetil combined with entecavir can effectively improve the liver function renal function of patients with HBV-GN and enhance the clinical curative effect.It is worth popularization and application.
