标准分法在医疗质量评价中的应用

摘　　要：目的应用标准分法对临床科室进行效益综合评价,为医院管理中有关综合评价的问题提供方法。方法参考高考标准分制度设置理念,将某院18个临床科室2013年-2014年度6项效率效益指标的原始值换算为标准分数,分别衡量各项指标水平及发展情况,利用象限分析法进行综合定位评价。结果妇产科、骨科、消化内科、眼科等位于第1象限,属效率效益水平相对较高且发展相对较快科室,其中妇产科效率效益水平最高,而骨科发展趋势最好;泌尿外科、中医科、神经内科等位于第2象限,效率效益水平相对较低但发展相对较快;急救部、呼吸科、血液科等位于第3象限,效率效益水平相对较低且发展相对较慢,其中急救部效率效益水平最低;普通外科、耳鼻喉科、心脏中心、神经外科等位于第4象限,效率效益水平相对较高但发展相对较慢,其中神经外科发展趋势最差。结论标准分法与象限分析法的结合运用可精确定位各科室效率效益在医院总体的动态位置,结果直观、可靠。Objective To use the standardized score method for comprehensive evaluation of clinical departments,in order to provide methods for the issues related to comprehensive assessment of hospital management. Method According to standardized score system concept of college entrance examination,the original values of the conversion efficiency of six efficient indicators of 18 clinical departments in a hospital from 2013 to 2014 standard scores were analyzed, levels of the indicators and development situation are measured respectively. Through using quadrant analysis methods, integrated positioning evaluation is done. Result Department of Obstetrics and Gynecology,Department of orthopedics, Department of internal medicine, Department of Ophthalmology, locates in the first quadrant, the level of efficiency is relatively high and relatively fast development. The highest level of efficiency and effectiveness of Obstetrics and Gynecology and the development trend of the best Department of orthopedics.Department of Urology, Department of traditional Chinese medicine, Department of internal medicine in the second quadrant, the level of efficiency is relatively low but relatively fast development. Emergency department,Department of respiration, Department of Hematology in the third quadrant, the level of efficiency is relatively low and the development is relatively slow, the efficiency of the lowest level of Emergency department. Department of general surgery, Department of ENT, Department of Heart center, Department of Neurosurgery in the fourth quadrant, the level of efficiency is relatively high but the development is relatively slow, the development trend of the Department of Neurosurgery. Conclusion Method combining with the use of standardized scores with quadrant analysis can pinpoint the efficiency and effectiveness of clinical departments in the hospital-wide dynamic location,and the result is intuitive and reliable.
