不依赖胸腔镜的前外侧小切口在难治性自发性气胸手术中的应用

摘　　要：目的：总结不依赖胸腔镜的前外侧小切口在治疗难治性自发性气胸手术中的应用经验。方法对18例难治性自发性气胸患者通过前外侧小切口进行手术，对其临床资料及手术方式进行分析。结果11例患者行单纯肺楔形切除，2例患者行同期双侧肺楔形切除加肺减容术，5例患者包括肺叶切除。手术时间40-280 min，平均127.3 min，围手术期无死亡。术后随访6-18个月，无复发病例。结论不依赖胸腔镜的前外侧小切口在难治性自发性气胸手术中具有较高应用价值。Objective To summarize the clinical experience of a mini-incision without thoracoscopic assisted for the operation to re-fractory spontaneous pneumothorax. Methods The clinical data and surgical procedures of 18 patients with refractory spontaneous pneumo-thorax who underwent surgery in our hospital were analyzed. Results Among the cases,11 patients underwent wedge resection,2 patients underwent bilateral surgery and the other 5 patients received lobectomy. The operation time was 40 - 280 min,with a a mean of 127. 3 min. All of the patients underwent surgery without perioperative death. They were followed up for 6 to 18 months and no recurrent pneumothorax was found. Conclusion The operation with a mini-incision without thoracoscopic assisted is a safe and effective procedure for elderly pa-tients with refractory spontaneous pneumothorax.
