D-二聚体在不同年龄孕妇中的表达情况及其临床价值分析

摘　　要：目的：研究D-二聚体在不同年龄孕妇中的表达情况及其临床价值。方法：选取梓潼县人民医院2012年1月至2014年1月接诊的180例孕妇作为研究对象,按孕妇年龄的大小分为A组（19～24岁）、B组（25～34岁）和C组（35～40岁）,各60例。调查分析3组孕妇妊娠晚期血D-二聚体浓度,并比较A组和C组孕妇的D-二聚体阳性率以及产后出血率。结果：B组和C组妊娠晚期血D-二聚体浓度均高于A组,差异具有统计学意义（P〈0.05）;且C组的血D-二聚体浓度[（1.91±0.67）mg·L^-1]明显高于B组[（1.68±0.65）mg·L^-1]。C组妊娠晚期血D-二聚体阳性率为88.33%,A组则为73.33%,两组比较差异具有统计学意义（P〈0.05）。A组孕妇的产后出血率为3.33%,C组为13.33%,A组明显低于C组,差异具有统计学意义（P〈0.05）。结论：孕妇体内D-二聚体的含量随妊娠年龄的增长而升高,其变化规律有助于了解不同年龄孕妇的纤溶活性;孕妇D-二聚体的含量检测对产后出血的预防、弥散性血管内凝血（DIC）等疾病的预测和判断以及患有糖尿病、高血压孕妇病情的监控具有重要的临床意义。Objective： To study the expression of D-dimer in pregnant women of different ages and clinical value of D-dimer. Methods： 180 cases of pregnant women in our hospital from January 2012 to January 2014 were studied,and they were divided into group A（ 19 to 24 years old）,group B（ 25 to 34 years old） and group C（ 35 to 40 years old） with different age,60 cases each. Analyzed the blood D-dimer concentration of three group pregnant women at late pregnancy. Compared the positive rates of D- dimer and postpartum hemorrhage rates between groups A and C. Results： The blood D-dimer concentration of group B and group C at late pregnancy were higher than that in group A,the difference was statistically significant（ P〈0. 05）. And the D-dimer concentration in group C [（ 1. 91 ± 0. 67） mg·L^-1]was obvious higher than that in group B[（ 1. 68 ± 0. 65） mg·L^-1]. The blood D-dimer positive rate of group C in late pregnancy was 88. 33%,and it was 73. 33% in group A,the difference was significant（ P〈0. 05）. Postpartum hemorrhage rates of the group A was 3. 33%,however,group C was 13. 33%,and it was significantly lower in group A（ P〈0. 05）. Conclusion： D-dimer content of pregnant women increased when gestational age increased and the variation can help to understand its fibrinolytic activity of pregnant women in different ages. D-dimer content testing of pregnant women has important clinical value to prevention of postpartum hemorrhage and prediction and judgment of diffuse with intravascular coagulation（ DIC）and surveillance of pregnant women with diabetes or hypertension condition.
