心理干预辅助匹维溴铵治疗老年肠易激综合征的临床观察

摘　　要：目的观察口服匹维溴铵治疗的基础上心理干预对老年肠易激综合征(IBS)的影响。方法 98例老年IBS患者随机分为观察组和对照组各49例,观察组在口服匹维溴铵片的基础上实施心理干预,对照组仅口服匹维溴铵片,疗程均为6周。观察2组治疗前后各项IBS症状评分和IBS生活质量评价量表(IBS-QOL)评分的变化,应用抑郁自评量表(SDS)和焦虑自评量表(SAS)进行心理评估。结果观察组腹痛、大便次数变化、大便性状变化积分的变化率显著高于对照组(P<0.05);2组患者IBS-QOL的8个方面评分均有不同程度的改善,观察组烦躁不安、身体意象、健康忧虑3个方面评分的变化率显著高于对照组(P<0.05);观察组SDS和SAS评分的变化率均显著高于对照组(P<0.05)。结论在口服匹维溴铵治疗老年IBS的基础上,心理干预可以进一步提高疗效。Objective To observe the effect of psychological intervention combined with pinaverium bromide on the treatment of elderly patients with irritable bowel syndrome （IBS）. Methods A total of 98 elderly IBS patients were randomly divided into two groups. The observa- tion group was treated with pinaverium bromide plus psychological intervention, while the control group was only given pinaverium bromide. The treatment duration was 6 weeks in both groups. The IBS symptom scores and IBS - QOL were assessed before and after treatment, and the SDS and SAS were applied for psychological assessment. Results The score changing rates of abdominal pain, stool frequency and stool character in the observation group were significantly higher than those in the control group （ P 〈 0.05）. Eight aspects of the IBS - QOL were all improved after treatment, and the score changing rates of dysphasia, body image and health worry in the observa- tion group were significantly higher than those in the control group （P 〈 0.05）. The score changing rates of SDS, SAS in the observation group were significantly higher than those in the control group （P〈 0.05）. Conclusion Psychological intervention combined with pinaverium bromide can im- prove the efficacy in elderly IBS patients.
