针刺治疗椎-基底动脉供血不足的临床疗效及对血流动力学影响的随机对照研究

摘　　要：目的:探讨针刺治疗椎-基底动脉供血不足的临床疗效及对血流动力学的影响。方法:将80例椎-基底动脉供血不足患者随机分为治疗组和对照组,每组各40例。对照组予盐酸氟桂利嗪胶囊5mg,1次/天,睡前口服。治疗组给予针刺治疗,取穴:于氏头针"颞区"、风池、大椎、百会、太阳、头维、供血。两组均治疗21天后对两组患者治疗前后眩晕等临床症状、体征及对椎动脉(VA)、基底动脉(BA)的平均血流速度(Vm)变化情况进行评价。结果:治疗组愈显率为85%,对照组愈显率65.5%,组间比较具有显著性差异(P<0.05)。两组治疗后左侧椎动脉(LVA)、右侧椎动脉(RVA)、基底动脉(BA)的平均血流速度均较治疗前明显升高(均P<0.01);治疗后组间左侧椎动脉(LVA)、右侧椎动脉(RVA)、基底动脉(BA)的平均血流速度比较,具有显著性(均P<0.01)。结论:针刺能够改善椎-基底动脉供血不足患者临床症状,并能调节椎-基底动脉血流速度,是治疗椎-基底动脉供血不足的一种有效方法。Abstr act：Objective：To assess acupuncture treatment for vertebrobasilar insufficiency （ VBI） and the effect on hemodynamics.Methods：80 patients suffering VBI were randomly divided into a treatment group （40 cases） and a control group （40 cases）.The control group received Flunarizine Hydrochloride capsules 5mg, once a day, orally at bedtime.Acupuncture therapy was given to the treatment group,and the acupoints were Niequ （Extra）,GB20,DU14,DU20,EX-HN5,ST8 and Gongxue（Extra）.After 3 weeks, the clinical symptoms of dizziness, signs and the mean velocity （ Vm） of the vertebral artery （ VA） and the basilar artery （ BA） changes were evaluated before and after treatment .Res ults：Markedly effective rate in the treatment group was 85%,and markedly effective rate in the control group was 65.5%.The two groups had a significant difference （ P〈0.05 ）;after treatment, the mean velocity of the left and right vertebral artery and basilar artery significantly in-creased（P〈0.01）;the mean velocity of LVA,RVA and BA between two groups after treatment were signifi-cantly different （ P〈0.01 ） .Conclusion：Acupuncture can improve the clinical symptoms of patients suffering VBI, and can adjust the blood flow velocity of vertebral-basilar artery.
