超声刀痔切除术与PPH术治疗环状混合痔的疗效比较

摘　　要：目的探讨超声刀在环状混合痔切除术中应用的疗效和安全性。方法Ⅳ期环状混合痔140例,随机分为两组,各70例,分别采用超声刀痔切除术(超声刀组)和PPH术(对照组),记录手术时间、术中出血量、口服镇痛药物用量、住院时间、住院费用等,随访6个月,记录复发率。结果超声刀组和对照组的手术时间、术后24h口服镇痛药物用量、住院时间、住院费用、复发率的差异有统计学意义(P<0.05),两组术中出血量差异无统计学意义(P>0.05)。结论与PPH术比较,超声刀痔切除术具有手术时间短、术后疼痛较轻、住院时间短、费用低、复发率低等优点,值得广泛推广。Objective To evaluate the effect and safety of excisional hemorrhoidectomy in which the harmonic scalpel was used by comparing with stapled hemorrhoidopexy.Methods Patients with GradeⅣ hemorrhoids who were employed during the trial period were recruited and randomized into two groups：（1）Harmonic scalpel hemorrhoidectomy with 70cases（2）Stapled hemorrhoidopexy with 70 cases.The operating time,intraoperative blood,oral analgesics using,hospital stay,hospital cost and recurrence were recorded by following up for six months.Results A short operating time and hospital stay was observed in harmonic scalpel group compared with stapled group.Reduced oral analgesics requirement during postoperative 24 hand hospital cost was observed in harmonic scalpel group compared with stapled group（P〈0.05）.And the recurrence was lower in harmonic scalpel group than in stapled group（P〈0.05）.There was no difference between the two groups in intraoperative blood loss（P〉0.05）.Conclusion The harmonic scalpel hemorrhoidectomy is quick,painless and cheap technic.And it is associated with a reduced hospital stay and recurrence after operation.
