协同护理模式在直肠癌Miles术后护理中的应用

摘　　要：目的探讨协同护理模式(CCM)在腹会阴联合直肠癌根治(Miles)术后护理中的应用效果。方法选取120例直肠癌Miles术后患者作为研究对象,将其随机分为观察组和对照组,每组各60例。对照组患者给予常规护理,观察组患者给予CCM护理。应用焦虑自评量表(SAS)和抑郁自评量表(SDS)对两组患者的焦虑状态、抑郁状态发生率进行评价和比较,对两组患者的住院时间、会阴部手术创口愈合时间、术后并发症发生率进行观察和比较。结果观察组患者焦虑状态和抑郁状态的发生率均显著低于对照组(χ2=18.039、9.600,P<0.05);观察组患者的会阴部手术创口愈合时间和住院时间均显著短于对照组(t=5.387、7.644,P<0.05);观察组患者的造口狭窄、发热及恶心呕吐等术后并发症的发生率均显著低于对照组(χ2=4.821、5.886、5.175,P<0.05)。结论在直肠癌Miles术后护理中应用CCM,可有效缓解患者的焦虑、抑郁等负面情绪、促进会阴部手术创口的愈合、缩短住院时间、降低术后并发症发生率,有助于提高治疗和护理效果、减少患者的痛苦和治疗费用。Objective To observe and analyze the application effects of collaborative care model（CCM）in nursing after abdominal perineal radical operation for rectal cancer（Miles operation）for rectal cancer.Methods 120 cases of patients with rectal cancer after Miles operation were selected as the research objects and randomly divided into the observation group and the control group.60 cases were included into each group.The patients in the control group were treated with the conventional nursing,while the patients in the observation group were treated with the nursing of CCM.The incidences of anxiety,depression of the patients in the two groups employing Self-rating Anxiety Scale（SAS）and Self-rating depression scale（SDS）were assessed and compared.The hospitalization time,the healing time for perineal operation wound,the postoperative complications of the patients in the two groups were observed and compared.Results The incidences of anxiety and depression of the patients in the observation group were significantly lower than those in the control group（χ^2=18.039,9.600,P〈0.05）;The hospitalization time and the healing time for perineal operation wound of the patients in the observation group were significantly shorter than those in the control group（t=5.387,7.644,P〈0.05）;The incidences of the postoperative complications such as stoma stenosis,fever and nausea and vomiting of the patients in the observation group were significantly lower than those in the control group（χ^2=4.821,5.886,5.175,P〈0.05）.Conclusion The application of CCM in nursing after Miles operation for rectal cancer can effectively alleviate the patients＇ negative emotions of anxiety or depression,promote the healing of perineal operation wound,shorten the hospitalization time,reduce the incidence of postoperative complications,help to improve the curative effects and the nursing effects,and reduce the pain and the cost of the patients.
