鼻内镜下两种腺样体手术方式的疗效比较

摘　　要：目的比较鼻内镜下低温等离子刀切除腺样体和鼻内镜下电动吸切器切除腺样体的手术效果。方法 44例腺样体肥大的患儿,其中21例采用鼻内镜下低温等离子刀切除腺样体术式(等离子刀组),23例采用鼻内镜下电动吸切器切除腺样体术式(吸切器组)。比较2组手术时间、术中出血量、假膜脱落时间、术后疼痛程度。随访患儿手术前后听力改善情况,采用儿童阻塞性睡眠呼吸暂停疾病特异性生活质量调查表(OSA-18)评估打鼾症状。结果 2组手术时间、出血量、假膜脱落时间、术后疼痛程度方面有显著差异(P<0.01);术后3个月随访,2组鼻塞、鼻漏、睡眠打鼾及听力改善情况无显著差异(P>0.05)。所有病例均无腺样体残体。结论鼻内镜下低温等离子刀切除腺样体能显著缩短手术时间,减少出血量,减轻术后疼痛和缩短伤口恢复时间。Objective To compare the effect between low- temperature plasma of endo- scopic adenoidectomy knife and cut electric suction device in treatment of patients with endoscopic adenoidectomy. Methods In 44 children with adenoidal hypertrophy, 21 children with low- tem- perature plasma of endoscopic adenoidectomy surgical knife were designed as plasma cutter group, and 23 children with electric suction cutting endoscopic adenoidectomy surgical device were designed as suction cutting device group. Operation time, blood loss, pseudo membranous off time and post- operative pain intensity were compared between two groups. Improvement situation of hearing in children was followed up after surgery, and children with obstructive sleep apnea was evaluated by the disease-specific quality of life questionnaire （OSA- 18）. Results There were significant differences in operation time, amount of bleeding, pseudo membranous off time and postoperative pain between two groups （P〈0.01）. After 3 months follow- up, there were no significant differences in two nasal congestion, rhinorrhea, snoring and hearing improvement between two groups （ P 〉 0.05）. all patients had no residual adenoids. Conclusion Endoscopic resection of low- tempera- ture plasma cutter adenoid fitness can significantly shorten the operation time, reduce the amount of bleeding, reduce postoperative pain and shorten recovery time.
