经皮肾镜钬激光碎石术治疗肾铸型结石疗效观察

摘　　要：目的探讨经皮肾镜钬激光碎石术治疗肾铸型结石的临床效果。方法 48例肾铸型结石患者分为观察组和对照组,每组24例。对照组患者给予开放性肾切开取石术,观察组患者给予经皮肾镜钬激光碎石术,比较2组患者结石清除率、手术时间、住院时间及术后并发症发生率。结果观察组和对照组患者结石清除率分别为96.78%和75.00%,观察组患者结石清除率高于对照组(P<0.05)。观察组患者术中出血量和术中输血量少于对照组,手术时间和住院时间显著短于对照组(P<0.05)。观察组和对照组患者并发症发生率分别为12.50%和41.67%,观察组患者并发症发生率低于对照组(P<0.05)。结论经皮肾镜钬激光碎石术治疗肾铸型结石疗效确切,术中出血量少,手术时间短,结石清除率高,术后并发症发生率低。Objective To explore the clinical effect of percutaneous nephrolithotomy holmium laser lithotripsy for trea- ting kidney staghorn stone. Methods A total of 48 patients with kidney staghorn stone were divided into observation group and control group,24 cases in each group. The patients in control group were treated with nephrolithotomy,but the patients in obser- vation group were treated with percutaneous nephrolithotomy holmium laser lithotripsy. The stone clearance rate, operation time, hospitalization time and postoperative complication incidence rate were compared between the two groups. Results The stone clearance rate in observation group and control group was 96.78% and 75.00% respectively,the stone clearance rate in observation group was higher than that in control group（ P 〈 0.05 ）. The intraoperative bleeding and blood transfusion in obser- vation group were less than those in control group （ P 〈 0. 05 ）, and the operation time and hospitalization time in observation group were shorter than those in control group（P 〈 0. 05）. The incidence rate of complication in observation group and control group was 12.50% and 41.67% respectively, the incidence rate of complication in observation group was lower than that in control group（P 〈 0. 05 ）. Conclusion Percutaneous nephrolithotomy holmium laser lithotripsy for treating kidney staghorn stone can obtain preonunced effect, less intraoperative bleeding, short operation time, high stone clearance rate and low inci- dence rate of complication.
