结核感染T细胞检测在儿童骨关节结核病中的诊断价值

摘　　要：目的本研究通过比较结核感染T细胞斑点试验(tuberculosis infection of T cells spot test,T-SPOT.TB)、抗酸染色、结核菌培养、实时荧光定量PCR检测及结核菌素纯蛋白衍生物(purified protein derivative,PPD)试验,评价T-SPOT.TB在儿童骨关节结核病中的诊断价值。方法选取36例诊断为骨关节结核的患儿,分别进行抗酸染色、结核菌培养、实时荧光定量PCR检测、结核菌素PPD试验及T-SPOT.TB,比较并分析检测结果。结果T-SPOT.TB阳性检出率显著高于抗酸染色、结核菌培养(P<0.05)。不同病变部位(脊柱、四肢关节)及不同年龄段间阳性检出率差异均无统计学意义(P>0.05)。结论 T-SPOT.TB可以作为儿童骨关节结核病的重要辅助诊断方法。Objective To compare the positive rate of tuberculosis infection of T cells spot test（T-SPOT.TB）with sputum smear,sputum culture,real-time polymerase chain reaction and purified protein derivative skin test in the diagnosis of osteoarticular tuberculosis in children.To investigate the value of T-SPOT.TB method for diagnosis of osteoarticular tuberculosis in children.Methods Thirty-six patients confirmed with osteoarticular tuberculosis were enrolled.The result of T-SPOT.TB was compared with four traditional test methods,and their clinical application value were further analyzed.Results The positive rate of T-SPOT.TB was significantly higher than that of sputum smear and sputum culture（P〈0.05）.But there were no statistical difference of different locations（spine or extremity joints）and age stages between groups.Conclusion T-SPOT.TB assay could provide useful aids in the diagnosis of osteoarticular tuberculosis of children.
