摘 要:目的探讨辐射力脉冲成像技术(acoustic radiation force impulse,ARFI)在甲状腺实性结节良恶性鉴别中的临床价值。方法选择甲状腺实性结节患者112例165个结节,健康体检者(对照组)40例,应用ARFI测量剪切波速度(shear wave velocity,SWV),绘制ROC曲线图。结果对照组SWV平均值为(2.20±0.80)m/s,良性结节的SWV平均值为(2.07±0.81)m/s,恶性结节的SWV平均值为(4.39±2.78)m/s,恶性组SWV高于对照组和良性组(P<0.01)。通过ROC曲线得出良、恶性结节SWV的临界值为2.76m/s,其敏感度为56.76%,特异度为85.16%,准确度为78.79%。结论 ARFI可定量分析甲状腺实性结节硬度,为甲状腺结节的良恶性鉴别提供了辅助方法。Objective To evaluate the value of acoustic radiation force impulse(ARFI)imaging in differentiating benign and malignant thyroid nodules.Methods The study involved112 patients with 165 thyroid solid nodules,and healthy subjects from body check 40 people in control group,ARFI was used to determine shear wave velocity(SWV)of all nodules.ROC curve was drawn in order to get the cutoff value.Results The mean SWV in control group and benign group was(2.20±0.80)m/s,(2.07±0.81)m/s,and the mean SWV in malignant group was(4.39±2.78)m/s,the malignant group was significantly higher than the benign or control group(P〈0.01).The cutoff value of SWV for benign and malignant nodules was 2.76 m/s,the sensitivity,specificity and accuracy were respectively 56.76%,85.16%and 78.79%.ConclusionARFI can provide the quantitative analysis in thyroid solid nodules,it may become a useful method in identification of benign and malignant thyroid nodules.
