血、尿、腹水Ⅲ型前胶原肽联合检测在肝脏疾病中的应用价值

摘　　要：目的:探讨血、尿、腹水Ⅲ型前胶原肽联合检测在肝脏疾病中的应用价值。方法:收集本院收治的经临床症状结合影像学及实验室检查等确诊为肝炎后肝硬化及肝硬化失代偿期的72例患者作为本次研究的观察组,对照组血尿取自本院健康体检者共60例,对照组腹水取本院收治的结核性腹膜炎腹水患者共60例,采用放射免疫法对所有的血、尿、腹水中的Ⅲ型前胶原肽进行检测,并对比分析两组结果。结果:观察组的血、尿、腹水的Ⅲ型前胶原肽含量明显高于对照组,肝硬化患者腹水中的Ⅲ型前胶原肽含量明显高于血、尿中的Ⅲ型前胶原肽含量,差异均具有统计学意义(P<0.05)。Ⅲ型前胶原肽检测敏感性、特异性、真实性比较均为腹水>血>尿。结论:肝硬化患者血、尿、腹水中Ⅲ型前胶原肽的检测均有较高的敏感性及特异性,Ⅲ型前胶原肽联合检测可有效反应患者肝纤维化、肝功能损伤及肝细胞坏死等方面的动态变化,在临床上具有一定的应用价值。Objective： To study the application value of combined detection of type Ⅲ procollagen peptide in blood, urine and ascite in patients with hepatic disease. Methods： 72 cases of patients with cirrhosis in decompensation period were en- rolled as study group. 60 healthy volunteers and 60 patients with tuhereulous peritonitis were also enrolled. Radio-immunity approach was adopted to detect of type Ⅲ procollagen peptide in blood, urine, ascite in order to compare these indexed among the three groups, flesults： Observation Contents of type Ⅲ procollagen peptide in blood, urine and ascite were significantly higher than that of the control group. Contents of type Ⅲ procollagen peptide in ascite of patients with patients with cirrhosis was significantly higher than that of blood and urine, the differences were statistically significant （P〈 0.05）. Sensitivity, specificity, authenticity of the type Ⅲ procollagen peptide detection in ascites were higher than that in the blood and urine. Conclusion： Combined detection of type gl procollagen peptide in blood,urine and ascite in patients with cirrhosis has high sensitivity and specificity, type combination of procollagen peptide detection can reflect the dynamic changes of patients with liver fibrosis and liver function damage and the liver cell necrosis, etc. It has certain application value in clinic.
