血液透析联合血液灌流治疗对糖尿病肾病患者营养状态及微炎症状态的影响

摘　　要：目的:研究血液透析联合血液灌流治疗对糖尿病肾病患者微炎症及营养状态的影响。方法:选择在本院接受住院治疗的糖尿病肾病患者118例作为研究对象,随机分为两组,观察组接受血液透析联合血液灌流治疗,对照组仅接受血液透析治疗。比较两组营养状态指标、生化指标以及炎症反应程度。结果:(1)营养状态:与对照组比较,观察组血清中白蛋白(Hb)和前蛋白(Alb)含量呈升高趋势,上臂围(MAC)、上臂肌围(MAMC)呈增加趋势;(2)生化指标:与对照组比较,观察组IGF-1、PA、TRF、RBP的mRNA含量较高;(3)炎症反应程度:与对照组比较,观察组血清中白介素-17(IL-17)、白介素-22(IL-22)、白介素-1β(IL-1β)、转化生长因子-β1(TGF-β1)、血清淀粉样蛋白A(SAA)水平呈降低趋势。结论:血液透析联合血液灌流治疗可以有效降低糖尿病肾病患者的微炎症状态,提升全身营养状态。Objective To study the effect of hemodialysis combined blood perfusion on micro inflammation and nutrition status of diabetic nephropathy patients. Methods： A total of 118 cases of diabetic nephropathy received treatment in our hospi- tal from December 2012 to December 2014 were selected for study and randomly divided into two group. Observation group re- ceived hemodialysis combined blood perfusion, control group received pure hemodialysis therapy. Then nutrition status index, biochemical index and inflammation response degree were compared. Results： Compared with control group, serum Hb and Alb levels of observation group showed increasing trend, MAC and MAMC showed ncreasing trend. Compared with control group, serum mRNA levels of IGF-1, PA, TRF, RBP of observation group showed increasing trend. Compared with control group, serum IL-17, IL-22, IL-1β,TGF-1β, SAA levels of observation group showed decreased trend. Conclusion= Hemodialy- sis combined blood perfusion treatment can effectively reduce inflammation in patients with diabetic nephropathy, and can improve general nutritional status.
