中药浴结合康复训练对脑卒中患者下肢恢复的影响

摘　　要：目的 :观察中药浴结合康复训练对脑卒中患者下肢痉挛、表面肌电信号变化,探讨其对缓解痉挛的临床疗效。方法 :56例脑卒中下肢痉挛患者随机分为治疗组和对照组各28例。治疗组采用局部中药浴结合康复训练治疗,对照组常规康复训练治疗。分别在治疗前、治疗8周时测定两组患者健患侧腓肠肌表面肌电均方根值(RMS),同时进行患肢改良Ashworth痉挛评分、下肢运动功能Fugl-Meyer评分。结果:治疗8周后,治疗组患者患侧腓肠肌RMS、改良Ashworth痉挛评分比较治疗前和对照组明显下降(P<0.05),下肢运动功能Fugl-Meyer评分与治疗前两组患者比较均有改善,治疗组改善明显高于对照组(P<0.05)。结论:中药浴结合康复训练能有效减轻脑卒中患者下肢痉挛,提高患者下肢运动功能。Objective：To observe the effect of Chinese herb bath combined with rehabilitation exercises on spasticity of lower limbs and changes of surface electromyography signals（SEMGs）,so as to explore its clini-cal effect on relieving spasticity. Methods：Fifty-six patients with spasticity of lower limbs caused by cerebral apoplexy were divided into treatment group and control group. Each group had 28 patients. The treatment group was treated by local Chinese herb bath and rehabilitation exercises. The control group was treated by conventional rehabilitation exercises. The root-mean-square（RMS） of SEMGs of gastrocnemius,score of modi-fied Ashworth scale for patients with spasticity and Fugl-Meyer score of motor function of lower limbs were detected respectively before the treatment and the 8th week of the treatment. Result： Eight weeks after the treatment,compared with the control group,the gastrocnemius RMS of the affected side and score of modified Ashworth scale of patients in the treatment group decreased significantly （P〈0.05）. Compared with the Fugl-Meyer score before treatment,the Fugl-Meyer score of both the two groups improved,but the improvement of the treatment group was better than that of the control group （P〈0.05）. Conclusion：Chinese herb bath com-bined with rehabilitation exercises can effectively relieve the spasticity of lower limbs of patients with cere-bral apoplexy and improve the motor function of lower limbs of the patients.
