摘  要:目的:探讨内皮微颗粒作为早期急性心肌缺血评估指标的可能性。方法:将24只雄性SD大鼠随机分为假手术组(Sham组)8只、心肌梗死组(MI组)8只和心肌缺血再灌注组(I/R组)8只,采用冠状动脉结扎制作急性心肌缺血大鼠模型。分别在造模后2 h和24 h采外周血检测肌酸激酶同工酶(creatine kinase-MB,CK-MB),肌钙蛋白T(cardiac troponin T,c TNT),内皮微颗粒(endothelial m Icroparticles,EMP),其中循环EMP用流式细胞仪检测。通过末端标记法(terminal-deoxynucleoitidyl transferase mediated nick end labeling,TUNEL)检测心肌细胞凋亡情况。结果:造模后2 h与24 h,I/R组大鼠EMP水平均显著低于MI组(P<0.05),造模后2 h MI组、I/R组大鼠的心肌细胞凋亡率均明显高于Sham组(P<0.05)。各组间比较,I/R组大鼠的c TNT表达水平造模后24 h较造模后2 h显著下降,差异有统计学意义(P<0.05);MI组大鼠的CK-MB表达水平造模后24 h较造模后2 h显著性升高(P<0.05);而EMP与心肌细胞凋亡率各组间比较差异无统计学意义。结论:内皮微颗粒可以作为早期急性心肌缺血的评估指标。Objective: To investigate the possibility of endothelial microparticles( EMP) being used as evaluation index for early acute myocardial ischemia. Methods: Twenty-four male SD rats were randomly divided into sham operation group,myocardial infarction( MI) group,and ischemia reperfusion( I / R) group( n = 8each). The rat models with acute myocardial ischemia were prepared by coronary artery ligation. At 2h and 24 h after modeling,peripheral blood was taken to measure creatine kinase-MB( CK-MB),cardiac troponin T( c TNT),and EMP. The circulating level of EMP were measured by flow cytometry. The myocardial apoptosis was determined by terminal-deoxynucleoitidyl transferase mediated nick end labeling( TUNEL) assay.Results: At 2h and 24 h after modeling,the EMP level in I / R group was significantly lower than that in MI group( P〈0.05). At2 h after modeling,the myocardial apoptosis in MI group and I / R group was significantly higher than that in Sham operation group( P〈0. 05). The expression level of c TNT in I / R group significantly decreased at 24 h after modeling compared with that at 2h after modeling,with statistical difference( P〈0. 05). The expression level of CK-MB in MI group significantly increased at 24 h after modeling compared with that at 2 h after modeling( P〈0. 05). There were no statistical differences in EMP and cardiomyocyte apoptosis between different groups.Conclusion: EMP can be used as evaluation index for early acute myocardial ischemia.
