摘  要:目的:研究自体软骨细胞复合于人脐带Wharton胶支架对兔膝关节全层软骨缺损的修复效果。方法:经自体关节软骨细胞经体外培养后复合到制备人脐带Wharton胶取向支架内构建细胞-支架复合体,选取健康清洁新西兰兔23只,雌雄不拘,体重2.5.3.0kg,取滑车沟中下部制作全层软骨缺损模型后随机分成A、B和C组。A组(n=10):植入自体软骨细胞+人脐带Wharton胶取向支架复合体;B组(n=10):植入单纯人脐带Wharton胶取向支架;C组(n=3):不做任何处理正常兔。分别于术后3个月和6个月各处死后取材进行生物力学特性评估检测。结果:压痕实验显示在3个月时A和B组修复区组织刚度分别达到正常软骨的45.72%和25.25%。且A组刚度明显优于B组,均低于C组(P〈0.05);到6个月时各自达到正常软骨刚度的69.76%和35.14%,同样A组刚度明显优于B组,均低于C组(P〈0.05)且在同期个各组之间均有显著性差异(F=80.309,P〈0.05)。结论:体外培养的自体软骨细胞与人脐带Wharton胶复合在体内的微环境作用下修复软骨缺损效果良好。为软骨组织工程提供了一种新支架材料。Objective: To investigate the effect of autologous chondrocytes in human umbilical cord Wharton's jelly matrix scaffold stents on rabbit knee full thickness cartilage defect repair. Methods: Autologous chondrocyte after in vitro preparation of the composite to build cells from human umbilical cord Wharton's jelly matrix glue oriented scaffolds - stents complex, choose healthy and clean 23 rabbits, male or female, weighing 2.5-3.0 kg, take the lower pulley groove were randomly divided into A, B and C after making a full thickness cartilage defect model. Group A (n=10): autologous chondrocyte implantation+human umbilical cord Wharton's jelly matrix plastic composite scaffold orientation; group B (n=10): implanted human umbilical cord Wharton's jelly malaix simple plastic bracket orientation; group C (n=3): no process for normal rabbit. Were everywhere after 3 months and 6 months after the death of biomechanical properties derived assessment test. Results: The indentation experiments are displayed in three months organizing stiffness restoration area A and B, raspoctively 45.72% and 25.25% of normal cartilage and compare with group A, the stiffness was significantly better than in group B, and both were lower than in group C (P〈0.05); to 6 months when each reached 35.14% and 69.76% of normal cartilage stiffness, compare with group A, the stiffness was significantly better than in group B, and both were lower than in group C (P 〈0.05) and in the same period of months between the groups was significant difference (F=80.309, P〈0.05). Conclusion: Autologous cultured chondrocytos and human umbilical cord Wharton's jelly matrix scaffold composite repair cartilage defects to good effect in rive microenvironment for cartilage tissue engineering provides a new scaffold.
