口腔专科医院夜间急诊的就诊模式和流行病学特点

摘　　要：目的通过分析口腔专科医院夜间急诊病例的临床资料,总结夜间急诊患者的就诊模式和流行病学特点。方法收集北京口腔医院2012年3—4月每日16:00—次日8:00的口腔急诊病例4 639例,对患者的就诊时间、就诊病因、治疗及用药情况和医疗保险情况等进行统计分析。本研究结果与北京口腔医院2001—2002年的研究资料进行比较研究,应用χ2检验进行分析。结果成年组与未成年组、成年组与老年组就诊人数在各就诊时间段的比较差异均有统计学意义(P<0.05);不同病因在各年龄组患者就诊人数的比较差异均有统计学意义(P<0.05);夜间急诊时段中17%为非急症治疗;未成年组中37.1%为医疗保险患者,为各组中最低;与2001—2002年数据比较,夜间患者流量明显增加。结论急诊医疗资源需求不断增长,口腔夜间急诊患者流量呈现时段性变化,决策者在制定口腔急诊发展规划时需要考虑这些变化规律。Objective To analyze the general information and clinical data of patients attending the night-time dental emergency of a specialized Stomatological Hospital in order to find out the pattern of treatment and epidemiologic features of emergency patients. Methods A retrospective study of the 4 639 patients with dental complaints who visited the emergency department during 4 pm to 8 am from March to April in 2012 was conducted. All patients were divided into three groups according to age. The visiting time, chief complaints,, medications and insurance status were analyzed in each group. Variance analysis was performed to compare with the data of night-time dental emergency in our hospital in 2001-2002. Chi-square test was used to analyze the differences of two researches. Results There were significant differences in number of visits among adult group, underage group. and elder group. Chief complaints of patients were significant differences in different groups. 17% of dental treatment were unemergency treatment in night-time emergency. Basic social medical insurance was 37. 1% in underage group, which was the lowest among three group. Numbers of patients in night-time emergency in 2012 were significantly higher than those in 2001- 2002. Conclusion The demand of emergency medical services is growing. Numbers of patients present changes in different periods at night-time dental emergency.Considerationshould be taken with the changes when make the dental emergency development planning.
