摘 要:目的:探讨早期有效抑制免疫反应对重症支原体肺炎治疗的有效性。方法:通过回顾性分析,收集2005年1月-2014年12月广州医科大学附属第二医院儿科诊断为肺炎支原体感染的2959例住院患者,其中诊断为重症支原体肺炎共112例,78例早期使用大坏内酯类抗支原体感染并联合使用激素治疗为观察组,与早期未使用激素的对照组44例的疗效、病程、并发症进行比较,以评价早期使用激素有效抑制免疫反应对重症支原体肺炎治疗的有效性。结果:观察组治疗7天显效、缓解及无效的病例数分别为52例(66.7%)、18例(23.1%)及8例(10.3%)。对照组显效、缓解及无效的病例数分别为12例(27.3%)、13例(29.5%)及19例(43.2%),两组比较,差异有统计学意义(P<0.05)。观察组平均住院天数(12.5±4.4)d与对照组(19.8±7.5)d比较,差异有统计学意义(P<0.05)。结论:早期有效抑制免疫反应治疗重症支原体肺炎可迅速控制病情,缩短住院时间,减少并发症的发生。Objective To investigate the therapy effect of mycoplasma pneumonia by early and effective immune reaction suppression. Methods By retrospectively analyze, 2959 hospitalized patients from the Second Affiliated Hospital of Guangzhou Medical University from 2005.1 to 2014.12, a total of 112 cases were diagnosed as severe mycoplasma antiinflammatory, including 78 cases in the early use of macrolide of treatmenting mycoplasma infection, also the early use of hormones to suppress the immune response compared with the control group without early usage of hormones. In order to evaluate the effectiveness of early effective immunosuppression therapy in the treatment of severe mycoplasma pneumonia the he curative effect, the course of disease, complications were compared. Results In the 78 cases of early stage of immunosuppression therapy, after 7 days therapy, 52 cases had valid effect accounted for 66.7 %. 18 cases relieved accounted for 23.1 % and 8 cases had no effect 10.3%. Without early immune control group of 44 cases, after 7 days treatment, valid 12 cases accounted for 27.3 %, 13 cases relieved 29.5 % and 19 cases had no effecty 43.2 %. The difference was statistically significant by chi square test(P 0.05). The early treatment of immunosuppressive agents, the mean disease duration was(12.5 ± 4.4)days, the control group was(19.8 ± 7.5)days, the difference was statistically significant(P 0.05). Conclusion Early effective suppression of the immune response in the treatment of severe mycoplasma pneumonia can shorten the hospitalization time and rapidly control the disease and reduce complications.
