摘 要:目的:研究Ax SYM和Architect i1000 sr系统测定地高辛血药浓度结果相关性,并比较其对内源性地高辛样免疫活性物质的敏感性。方法:同时采用Ax SYM和Architect i1000 sr系统测定100例患者的地高辛血药浓度,以Ax SYM的测定值为X,Architect i1000 sr的测定值为Y,进行线性回归,评价其相关性。同时采用两个系统测定56例未服用地高辛患者的表观地高辛血药浓度,比较其对内源性地高辛样免疫活性物质的敏感性。结果:Architect i1000 sr的测定值小于Ax SYM的测定值,两者相关性良好(r=0.931),回归方程为Y=0.746 X–0.315 8。56例未服用地高辛患者Architect i1000 sr测得的表观地高辛浓度均为0,Ax SYM测定时28例患者表观地高辛浓度大于0.3 ng·m L-1。结论:尽管Ax SYM和Architect i1000 sr系统测定地高辛血药浓度结果相关性良好,然而Architect i1000 sr系统的测定值偏低,进行方法间切换时,药师和医师应予以注意。Architect i1000 sr系统对内源性地高辛样免疫活性物质的敏感性低于Ax SYM系统。Objective: To assess the correlation between the serum digoxin levels measured by AxSYM and Architect il000 sr systems and compare the two systems with respect to interference from digoxin-like immunoreactive substances. Methods: Serum digoxin levels of 100 patients were determined by AxSYM and Architect i1000 sr systems. Regression analysis was performed to evaluate the correlation. The interference of digoxin-like immunoreactive substances on both assays was investigated in samples from 56 patients who were not taking digoxin. Results: Serum digoxin levels determined by Architect il000 sr system were significantly lower than those of AxSYM system. Concordance analysis revealed a positive and statistically significant correlation coefficient (r = 0.931). The equation was Y = 0.746 X- 0.315 8. The apparent digoxin concentrations were 0 in all of the non digoxin-treated patients when measured with Architect i1000 sr system, while the apparent digoxin concentrations of 28 non digoxin-treated patients were greater than 0.3 ng.mL-1 when measured with AxSYM system. Conclusion: The correlation between the serum digoxin levels from both assays is well. However, values obtained by Architect il000 sr system are lower than those of AxSYM system. Susceptibility to interference from digoxin-like immunoreactive substances of AxSYM system was significantly greater than that of Architect i1000 sr system.
