32例龙胆泻肝丸引起肾损害的不良反应分析

摘　　要：目的：了解龙胆泻肝丸引起肾损害的情况,为临床合理用药提供参考。方法：采用回顾性方法,对我院2004–2014年因服用龙胆泻肝丸引起肾损害的药品不良反应报表进行分析提取,共计32例。对患者的性别、年龄、用药疗程、剂量与原发疾病关系、因果关系评价及严重程度、实验室检查及其他检查、ADR临床表现、治疗方法等进行分析。结果：32例报表均评定为可能,严重ADR中女性占81.25%;平均年龄为（54.72±8.61）岁;平均服用药物史为（6.55±4.48）年,服药时间0.2~17年;Scr平均（532.2±246.7）μmol·L-1;BUN（120.80±6.80）mmol·L-1;Hb（87.59±18.34）g·L-1;以脾肾气阴两虚型为主要临床表现,采用中西医结合方法治疗,好转率达90.63%。结论：含有关木通成方的龙胆泻肝丸引起肾损害人数不可估量;患者年龄集中在50~69岁的中老年人,女性多于男性;服用时间长短、剂量与肾功能减退程度、贫血程度不完全平行,个体差异较大,均为不可逆性肾功能受损。Objective： To analyze the adverse drug reaction of kidney injury induced by Longdanxiegan pills and provide reference for clinical rational use of drugs. Methods： According to WHO evaluation criterion, a total of 32 cases of kidney injury induced by Longdanxiegan pills in our hospital from 2004 to 2014 were collected and analyzed in respect of age and gender distribution, duration and dose of drug administration, primary disease, causality, laboratory tests and other tests, ADR clinical manifestation, therapeutic method, etc. Results： The evaluation of 32 cases were possibility, including 26 female cases （81.25%） in severe ADRs. The average age was （54.72 ~ 8.61） years. The average medication duration was （6.55 ~ 4.48） years （from 0.2 to 17 years）. The average Scr was （532.2 ±246.7） μmol.L-1, BUN was （120.80 ± 6.80） mmol.L-1, Hb was （87.59 ± 18.34） g.L-1. The main clinical manifestations were deficiency of both qi and yin in spleen and kidney. The recovery rate was 90.63% with integrated traditional Chinese and western medicine treatment. Conclusion： The cases of kidney injury induced by Longdanxiegan pills containing manchurian dutchmanSpipe stem were immeasurable. The age range of the patients was from 50 to 69 years. The number of female patients was more than that of male patients. The medication duration and dosage of drug administration were not on a completely parallel with the degree of renal function impairment and anemia. There were individual differences in the cases of kidney irreversible injury induced by Longdanxiegan pills.
