摘　　要：目的比较注射用瑞替普酶(r-PA)与注射用尿激酶(UK)在急性ST段抬高型心肌梗死溶栓治疗中的应用效果。方法选取2011—2013年中国人民解放军第三一六医院心内科收治的首次急性ST段抬高型心肌梗死患者50例,随机分为r-PA组(24例)与UK组(26例)。r-PA组患者予以r-PA溶栓治疗,UK组患者予以UK溶栓治疗。观察两组患者溶栓治疗情况(血管再通率、病死率、出血发生率)及不良反应发生情况。结果 r-PA组患者血管再通率高于UK组,出血发生率低于UK组,差异有统计学意义(P<0.05),两组患者病死率比较,差异无统计学意义(P>0.05);r-PA组患者不良反应发生率低于UK组,差异有统计学意义(P<0.05)。结论 r-PA在急性ST段抬高型心肌梗死溶栓治疗中的应用效果好于UK,可提高血管再通率,降低出血发生率,且不良反应少。Objective To compare the effect of between reteplase for in jection （r-PA） and umkinase for in jection （UK） in thrombolytic therapy for acute myocardial infarction with ST - segment elevation. Methods A total of 50 patients with acute myocardial infarction with ST - segment elevation were selected in the 316th Hospital Chinese People＇s Liberation Army from 2011 to 2013, they were randomly divided into r-PA group （24 cases） and UK group （26 cases）, r-PA group was given r-PA treatment, UK group was given UK treatment. Thrombolysis treatment outcome （ vessel repass rate, mortality rate, bleed- ing rate） and incidence of adverse reactions between the two groups were observed. Results Vessel repass rate of r-PA group was higher than that of UK group, bleeding rat of r-PA group was lower than that of UK group （ P 〈 0. 05 ）, mortality rate showed no significant differences between the two groups （ P 〉 0. 05 ） ; incidence of adverse reactions of r-PA group was lower than that of UK group （P 〈 0. 05）. Conclusions r-PA thrombolytic in acute myocardial infarction with ST - segment elevation the application effect is better than UK, can improve the rate of vessel pepass, reduce the incidence of bleeding and has less ad- verse reactions.
