2012-2014年产吲哚金黄杆菌的临床分离情况及耐药性变迁

摘　　要：目的分析本院临床分离产吲哚金黄杆菌的分布及耐药性变迁,指导临床合理用药。方法对本院2012年1月至2014年12月期间临床分离的产吲哚金黄杆菌用VITEK2Compact全自动微生物分析仪进行鉴定和药敏试验,根据美国临床和实验室标准协会(CLSI)提供的2013年MIC标准判断结果。结果3年中收集了32株产吲哚金黄杆菌,68.8%为60岁以上的患者,菌株分离标本来源主要为尿液。32株菌均呈现多重耐药。对头孢菌素类抗生素的耐药率很高,特别是对头孢唑啉和头孢替坦的耐药率均为100.0%。对复方新诺明的耐药率最低。分析3年分离出的产吲哚金黄杆菌的耐药性,发现该菌对亚胺培南和复方新诺明的耐药率有所下降,对其他抗生素的耐药率无明显变化。结论分离的产吲哚金黄杆菌呈现多重耐药,应参照药敏结果合理选用抗菌药物。Objective To investigate the distribution and antimicrobial resistance of Chryseobacterium indologenes,and provide guidance for rational use of antibiotics.Methods Chryseobacterium indologenes strains isolated from patients in our hospital from January 2012 to December 2014 were identified.The Minimum inhibitory concentration of Chryseobacterium indologenes strains was detected by VITEK2 Compact automatic identification system.The results were analyzed according to the guidelines of CLSI（2013edition）.Results Thirty-two strains of Chryseobacterium indologenes were isolated,68.8% of which came from patients above 60 years old.Most of the strains were isolated from the urine specimens.All the strains were multidrug resistant.The resistance rates of the 32 strains to cephalosporins were very high,which was up to100%to both cefazolin and cefotetan.The drug-resistance rate to compound sulfamethoxazole was the lowest.The results showed that the drug-resistance rates of these strains to imipenem and compound sulfamethoxazole had reduced in the three years,while their resistance rates to other antibiotics had no significant changes.Conclusion In our hospital,Chryseobacterium indologenes present multi-drug resistance.Antimicrobial treatment for Chryseobacterium indologenes infection should be guided by results of drug sensitivity test.
