全椎板切除减压对老年退行性腰椎管狭窄症的疗效分析

摘　　要：目的：探讨老年退行性腰椎管狭窄症行全椎板切除减压及椎弓根钉内固定手术治疗的效果。方法：回顾分析自2008年04月-2011年04月我院手术治疗的32例60岁以上的退行性腰椎管狭窄症患者临床资料。其中男19例,女13例,年龄60~77岁,平均67.4岁。均采用广泛椎板切除减压、椎弓根螺钉固定及椎间植骨融合术治疗,采用VAS评分、JOA评分和Oswestry评分标准进行疗效评估。结果：随访6个月~4年,平均随访22.7个月,VAS、JOA、Oswestry评分均有显著改善。优良率84.4%;手术改善率78.92%。结论：经全椎板切除减压可有效改善老年退行性腰椎管狭窄症的临床症状,获得满意疗效。Objective To analyze the curative effects of total laminectomy and internal pedicle screw fixation on senile degenerative lumbar spinal stenosis. Methods Retrospectively analyzed the clinical data of 32 senile degenerative lumbar spinal stenosis cases that underwent decompressive total laminectomy, internal pedicle screw fixation, and intervertebral bone graft fusion from April, 2008 to April 2011 in our hospital. 19 of the 32 cases were male and 13 were female with an average age of 67.4 years old ranging from 60 to 77. Clinical effects were analyzed by VAS、JOA and Oswestry evaluation systems.Results Follow up which lasted for an average of 22.7 months, ranging from 6 months to 4 years showed that the average VAS score was（5.01 ± 0.98） to（0.71 ± 0.69）,JOA scores were improved from preoperative（7.94 ± 1.93） points to postoperative（24.56 ± 2.33） points; and the Oswestry scores from preoperative（61.69 ± 6.58） points to postoperative（21.73 ± 6.15） points. 90.6 % of these patients expressed that their life qualities were obviously improved after the treatment, and the continuous walk distances were also significant longer than before. Conclusion Decompressive total laminectomy, internal pedicle screw fixation, and intervertebral bone graft fusion can obviously relive the symptoms of senile degenerative lumbar spinal stenosis with satisfying curative effects.
