多种肿瘤标志物检测对食管鳞癌的诊断及意义

摘　　要：目的探讨血清细胞角蛋白15(CK15)、骨形态发生蛋白2(BMP2)在食管鳞状上皮细胞癌(ESCC)患者中的表达和诊断价值。方法测定30例ESCC患者组和30例正常对照组血清中CK15、BMP2、细胞角蛋白19片段(CYFRA21-1)、鳞状上皮细胞癌抗原(SCC)、增殖细胞核抗原(PCNA)、癌胚抗原(CEA)水平。CK15、BMP2、SCC、PCNA采用酶联免疫吸附试验检测,CEA、CYFRA21-1采用全自动化学发光检测仪检测。结果 ESCC患者中血清CK15水平低于正常对照(P<0.05),SCC、CEA、CYFRA21-1水平高于正常对照(P<0.05),BMP2和PCNA的血清水平与正常对照相比无显著差异(P>0.05)。CK15对ESCC诊断的敏感性和特异性分别为33.3%和90.0%,联合检测CK15和CYFRA21-1对ESCC诊断的的敏感性和特异性分别为60.0%和90.0%,联合检测CK15、SCC、CEA和CYFRA21-1对ESCC诊断的的敏感性和特异性分别为76.7%和70.0%。结论血清CK15的变化可能与ESCC的发生有关;联合检测CK15和SCC,CEA,CYFRA21-1对ESCC具有较大诊断价值;血清BMP2和PCNA对ESCC无诊断价值。Objective To explore the expression and its diagnostic value of CK15,BMP2,SCC,PCNA,CEA and CYFRA21-1 in serum of the patients with esophageal squamous cell carcinoma（ESCC）.Methods The CK15,BMP2,SCC,PCNA,CEA,CYFRA21-1 levels of 30 patients with esophageal squamous cell carcinoma and 30 normal controls were detected.The CK15,BMP2,SCC and PCNA levels were determined by ELISA,and the CEA and CYFRA21-1 levels were detected by fully automatic chemiluminescence detector.Results The serum level of CK15 in esophageal patients was significantly lower than those in controls（P ＜ 0.05）.The serum levels of SCC,CEA and CYFRA21-1 in esophageal patients were significantly higher than those in controls（P ＜ 0.05）.The sensitivity and specificity of CK15 in the diagnosis of esophageal cancer were 33.3％ and 90.0％,respectively.The sensitivity and specificity of combined detection of CK15 and CYFRA21-1 were 60.0％ and 90.0％,respectively.The sensitivity and specificity of combined detection of CK15,SCC,CEA and CYFRA21-1 were 76.7％ and 70.0％,respectively.The serum levels of BMP2 and PCNA in patients with esophageal squamous cell carcinoma showed no significant difference from controls （P ＞ 0.05）.Conclusion The changes of serum CK15 may be related to the occurrence of ESCC.The combined detection CK15 and SCC,CEA,CYFRA21-1 marker has important values in the diagnosis of ESCC.The serum levels of BMP2 and PCNA have less significance in diagnosis of esophageal cancer.
