宁养院晚期癌症患者照顾者照顾反应与应对方式的相关性研究

摘　　要：目的了解宁养院晚期癌症患者照顾者的照顾反应水平与应对方式现状,并分析照顾者的照顾反应与应对方式的相关性。方法采用自制一般资料问卷、照顾者反应评估量表、简易应对方式问卷,对116例宁养院晚期癌症患者的照顾者进行调查。结果照顾者自尊维度得分(29.00±3.29)分,处于重度水平;时间安排受打扰、经济问题、健康问题、家庭支持缺乏4项维度得分分别为(13.08±3.07)分、(10.16±3.08)分、(18.62±3.29)分、(12.40±3.72)分,晚期癌症患者照顾者的照顾反应处于中、重度水平。自尊维度与积极应对方式呈正相关(r=0.307,P<0.01),健康问题(r=-0.287,P<0.01)、经济问题(r=-0.185,P<0.05)、家庭支持缺乏(r=-0.325,P<0.01)与积极应对方式呈负相关;健康问题(r=0.332,P<0.01)、时间安排受打扰(r=0.252,P<0.01)、经济问题(r=0.191,P<0.01)与消极应对方式呈正相关。结论积极应对方式有助于缓解宁养院晚期癌症患者照顾者的照顾负担,医护人员应给予照顾者照顾技巧指导,提高其照顾能力,促使其选择积极的应对方式,以达减轻其照顾负担。Objective To investigate the level of caregiver reaction and coping styles in caregivers of patients with advanced cancer, and to analyze the correlation between them. Methods With convenient sampling, 116 caregivers of patients with advanced cancer in the hospice care-center of Dalian were investigated with the Caregiver Reaction Assessment （CRA） and Simplified Coping Style Questionnaire. Results The score of self-esteem dimension was 29.00 ±3.29 and that of disrupted schedule, financial problems, health problems, lack of family support was 13.08 ±3.07, 10.16 ±3.08, 18.62 ±3.29, 12.40 ±3.72 respectively. Caregivers’ reaction remained in a medium or high level. Caregivers adopted positive coping style more frequently. Positive coping style was positively correlated with self-esteem（r=0.307, P〈0.01）, but negatively correlated with health problems （r=-0.287,P〈0.01）or financial problems （r=-0.185, P〈0.05）or lack of family support （r=-0.325,P〈0.01）. Health problems （r=0.332, P〈0.01）, disrupted schedule（r=0.252, P〈0.01）and financial problems（r=0.191,P〈0.01） were positively correlated with negative coping style. Conclusion Positive coping styles benefit reducing caregivers’ burden and improving the positive feelings, while negative coping style will aggravate caregiver burden. Therefore, the improvement of the ability to care would help caregivers take positive coping styles frequently.
