摘 要:目的分析唑来膦酸盐联合脉冲电磁场治疗绝经后骨质疏松症的临床疗效。方法将我院收治的63例绝经后骨质疏松症患者随机分为观察组21例,对照1组21例和对照2组21例。观察组患者行唑来膦酸盐联合脉冲电磁场治疗,对照1组患者行单纯的唑来膦酸盐治疗,对照2组患者行单纯的脉冲电磁场治疗。测量、评价并统计3组患者治疗前后的腰椎正位(L2-4)和右股骨颈骨密度(BMD)、疼痛程度(VAS)和日常生活能力(ADL),同时检测3组患者治疗前后的骨转换生物标记物,包括血清碱性磷酸酶(ALP)和血清骨钙素(OC)。结果治疗后6个月,观察组患者的平均腰椎正位(L2-4)及右股骨颈BMD值均高于对照1组和对照2组(P<0.05);治疗后6个月和12个月,观察组患者平均VAS和ADL评分均低于对照1组和对照2组(P<0.05);同时,治疗后12个月,观察组患者的平均ALP、OC指标值均低于对照组(P<0.05)。结论唑来膦酸盐联合脉冲电磁场治疗绝经后骨质疏松症的临床疗效显著,能有效提高绝经后骨质疏松症患者的骨密度和日常生活能力,快速缓解骨痛,且降低骨转换生物标记物水平,控制骨量减少。Objective To analyze the clinical efficacy of zoledronic acid combined with pulsed osteoporosis of electromagnetic therapy on postmenopausal osteoporosis. Methods Sixty-three patients with postmenopausal osteoporosis were randomly divided into the observation group( 21 cases),control group 1( 21 cases),and control group 2( 21 cases). The patients in observation group received zoledronic acid combined with pulsed electromagnetic field therapy. The patients in control group 1 received zoledronic acid treatment only. The patients in control group 2 received electromagnetic field treatment only. Bone mineral density( BMD) of the lumbar vertebrae( L2-4) and the right femoral neck was measured. Pain degree( VAS) and activity of daily living( ADL) were evaluated. Bone turnover markers,including serum alkaline phosphatase( ALP) and serum osteocalcin( OC),were detected in the 3 groups before and after the treatment. Results After 6-month treatment,BMD of L2-4 and the right femoral neck in observation group was higher than that in the control group 1 and control group 2( P〈0. 05). After treatment for 6 and 12 months,VAS and ADL scores of the patients in observation group were lower than those in the control group 1 and the control group2( P〈0. 05). After 12-month treatment,the average ALP and OC in observation group were lower than those in the control groups( P〈0. 05). Conclusion The clinical efficacy of zoledronic acid combined with pulsed electromagnetic fields on the treatment of postmenopausal osteoporosis is significant. It can effectively improve bone mineral density and daily life ability in postmenopausal osteoporosis patients,release the bone pain rapidly,and decrease the levels of bone turnover markers to control bone loss.
