摘  要:目的探讨2型糖尿病(T2DM)并阻塞性睡眠呼吸暂停综合征(OSAS)患者血糖波动与炎症反应相关性研究。方法选取入住我院的32例T2DM并OSAS患者作为观察组,32例T2DM患者作为对照组。检测两组患者血清IL-6、TNF-α、MCP水平,并利用动态血糖监测系统测量日平均血糖波动(MAGE)、血糖水平标准差(SDBG)、餐后血糖波动幅度(MPPGE)。结果观察组MAGE、SDBG、MPPGE水平均明显高于对照组(P<0.05)。观察组的炎症因子IL-6、TNF-α和MCP水平明显高于对照组(P<0.05)。Spearman相关分析示IL-6、TNF-α水平与MAGE、SDBG、MPPGE呈正相关(P<0.05),MCP水平与MPPGE呈正相关(P<0.05)。结论 T2DM合并OSAS患者血糖波动与体内炎症反应有一定相关性,及时有效的控制血糖波动,有可能是减轻炎症反应和缓解病情的关键。Objective To evaluate the association between glucose fluctuation and inflammation in patients with type 2 diabetes mellitus (T2DM) and obstructive sleep apnea syndrome (OSAS). Methods 32 T2DM patients complicated with OSAS were enrolled in this study as observation group, and another 32 patients only with T2DM were selected as the control group. The levels of serum IL-6, TNF-α and MCP in the two groups were measured and recorded. The dynamic blood glucose monitoring system was used to measure MAGE, SDBG and MPPGE. Results Compared with the control group, the observation group had higher levels of MAGE, SDBG, MPPGE, IL-6, TNF-α and MCP. Sperman relation analysis proved that the contents of IL-6 and TNF-α were positively associated with the levels of MAGE, SDBG and MPPGE ( P 〈 0.05 ). The level of MCP was positively related with MPPGE ( P 〈 0.05 ). Conclusion Glucose fluctuation has a certain correlation with inflammation in T2DM patients complicated with OSAS. Therefore, the effective and timely control of glucose fluctuation can alleviate inflammation injury.
